Purpose:To explore the comparative experimental study on the treatment ofacute myocardial infarction in rats through two transplantation ways ofintramyocardial injection of bone marrow mesenchymal stem cells and by acupointinjection of bone marrow mesenchymal stem cells, to find a treatment for acutemyocardial infarction.Material and method:1.The experimental materials and grouping:1.1Experimental animals: select60SD rats1.2Grouping:60rats were randomly divided into four groups named shamoperation group, model group, intramyocardial injection of stem cell group andacupoint injection stem cell group.1.3Molding method: This experiment used the method for coronary arteryligature of acute myocardial infarction model, the left coronary artery ligationin rats anterior descending, ECG: make II lead ST segment bow back up. Thespecific method is: cut longitudinally long about1.5cm skin tissue on the thirdin the left parasternal intercostal0.5cm with scissors, use forceps to clampthe external intercostal muscles and small vasculars, use little tweezers fromrib on both sides of the rib3clearance through the twin ligature, the ligationon both sides of the line of blunt separated, intercostal muscles and ligationintercostals arteries on both ends of the frame,3sides ligation line in themiddle of the cut3ribs gently pull enlarge eye shot to the right, open thepleura and pericardium, separate the thymus tissue, between the left atrium andpulmonary arterial cone, find the line in the myocardium and the main line tothe heart of the left anterior descending coronary artery (LAD) with noninvasivesuture needle in left heart through the myocardial surface below the ears of 2mm place, electrocardiogram (ECG) in pulmonary arterial cone branch needle,observation, for its stability returns after15min, ligation in the surfaceof the heart, a corresponding dark area on the surface of the heart showssuccessful cardiac ischemia model.2.Ways of transplantation:2.1Acupoint injection stem cell group: After successful molding of acutemyocardial infarction in rats,72hours later, through the four points ofbilateral heart Yu, Yang, Shan to the middle, inject bone marrow mesenchymalstem cells0.3ml in each acupoint.2.2Myocardial injection of stem cell group: After successful molding ofacute myocardial infarction in rats,72hours later, do the second open chestoperation, find six points in direct vessel blood supply area and its surroundingarea, in the left anterior coronary artery with micro injector, uniformlytransplantation in1*1010/L bone marrow mesenchymal stem cells in1.2mL, upliftlike a hill in myocardium means successful stem cells transplantation.3.Observation index:3.1Rat cardiac function: observe left ventricular end-diastolic diameter(LVEDD), left ventricular end systolic diameter (LVESD), left ventricular shortaxis shortening rate (FS) and ejection fraction (EF), test the average of threeconsecutive cardiac cycle and brain natriuretic peptide (BNP) to observe therat cardiac function.3.2To observe the hemodynamic changes in rats: observe the detection returnof he left ventricular peak pressure (LVSP), left ventricular end diastolicpressure (LVEDP), left ventricular isovolumic period of maximum pressure changerate (dp/dt), the rate of left ventricular pressure curve on {numerical changes(dp/dt), P-1}, blood pressure (BP), heart rate (HR) and cardiac mass index (heartmass index HMI;HMI, the heart weight/body weight), left ventricular mass index(left ventricular mass index, LVMI;left ventricular weight/LVMI=heart weight)to observe hemodynamic changes in rats.3.3Observation rat ventricular arrhythmia conditions: Stem cells after injection of7D,14d,28d ECG changes were observed between the animal, frequentventricular premature (>5beats/min), pair ventricular premature or shortparoxysmal ventricular tachycardia (>10beats/min), multifocal prematureventricular vulnerable period or fall in one stroke (R wave on the T wave) appearthe situation, monitoring time is8h.3.4Safety index of rats: Through the blood for blood, serum protein, serumenzymogram of rats, serum bilirubin, serum creatinine and blood urea nitrogenwere examined as safety index to understand the transplantation of bone marrowmesenchymal stem cells.Results:1.Cardiac function Results showed that after successful intramyocardialinjection of stem cell group and acupoint injection stem cell transplantationgroup, there is a trend towards improvement of each group, the plasma levelsof BNP and left ventricular end diastolic diameter (LVEDD), left ventricularend systolic diameter (LVESD), left ventricular fractional shortening (FS) andejection fraction (EF) were measured, the two groups of treatment group and modelgroup were compared with P<0.05, there was statistical significance, comparethe two groups of intramyocardial injection of stem cell group and acupointinjection stem cells with P>0.05, no statistical significance.2.The results of detection of hemodynamic showed that through the LVSP,+dp/dtmax,-dp/dtmax, LVEDP, HP, SAP, MAP, DAP, HR and determination of HMI, LVMI values,two groups of treatment group and model group were compared with P<0.05, therewas statistical significance, intramyocardial injection of stem cell group andacupoint injection stem cells between the two groups with P>0.05, no statisticalsignificance.3.Bone marrow mesenchymal stem ratio is P>0.05the occurrence of ventriculararrhythmias in cell transplantation group and model group, no statisticalsignificance.4.The comparison results between each group of rats blood routine, serum protein, serum enzymes, serum bilirubin, serum creatinine and blood urea nitrogen andother statistical returns P>0.05,no statistical significance.5.For the determination of return above indicators, myocardial injection of bonemarrow mesenchymal stem cells and acupoint injection of bone marrow mesenchymalstem cell transplantation for two groups of treatment of acute myocardialinfarction in rats, no significant difference between the two groups.Conclusion:1.Myocardial injection and acupuncture point injection of bone marrowmesenchymal stem cells for monitoring cardiac function in rats with acutemyocardial infarction have improving effects, no significant difference betweenthe two groups.2.Myocardial injection and acupuncture point injection of bone marrowmesenchymal stem cells for each indicator of acute myocardial infarction canimprove hemodynamic effect, no significant difference between the two groups.3.Myocardial injection and acupuncture point injection of bone marrowmesenchymal stem cell treatment did not induce ventricular arrhythmias.4.Myocardial injection and acupuncture point injection of bone marrowmesenchymal stem cell treatment method has a certain degree of security.5.Myocardial injection of bone marrow mesenchymal stem cells and acupointinjection between the two groups for acute myocardial infarction in rats hadno significant difference in treatment effect.
