Clinical Study On The Treatment Of Chronic Pulmonary Heart Disease (Qi Deficiency And Blood Stasis Syndrome) Based On The Theory Of "Collateral Disease"

Research Objective:To evaluate the efficacy and safety of Bu Yanghuanwu decoction in the treatment of chronic pulmonary heart disease with Qi deficiency and blood stasis syndrome. And to explore the clinical value and promoting significance of the decoction in treating chronic cor pulmonale in the view of collaterals disease theory of traditional Chinese medicine.Research Methods:simple randomized control method was adopted in this clinical trial. And 40 patients were collected from outpatient in department of lung internal medicine in Chengdu University of Traditional Chinese Medicine affiliated hospital. According to random number table method, all patients were divided into the cooperation of Chinese and western medicine treatment group (treatment group) and western medicine treatment group (control group) with 20 patients in each group. Patients in the control group were given conventional western medicine treatment (Controlling oxygen cure; Budesonide and Formoterol Fumarate Powder for inhalation(Symbicort Turbuhaler) 160/4.5ug inhale bid; Fudosteine Capsules 0.4g po tid), while patients in the treatment group were given Bu Yanghuanwu decoction+ conventional western medicine. To observe changes of TCM syndrome score, CAT score, Lung function score, blood rheology indexes, walking distance within 6 minutes between the two groups after treating for 4 weeks and acute exacerbation were followed up for 3 months.Results:(1) Comparison in syndrome curative effect shows that the total effective rate in the treatment group is 80%,while that is 65% in the control group, the difference was statistically significant (P<0.05).(2) TCM syndrome scores in the two groups shows that the difference was statistically significant(P<0.05),and it have significant difference of the group itself compared before and after treatment(P<0.01).(3) Comparison of single symptom and signs of TCM of the group itself shows that the difference was statistically significant (P<0.05 or P<0.01); the symptoms of shortness of breath, tiredness, sweating, scaring wind, incline to catch cold and purple tongue are improved more significantly in the treatment group than that in the control group (P<0.05), but it seems that there is no statistical significance in the remnant aspects (P>0.05).(4) CAT scores in the two groups shows that the difference was statistically significant(P<0.05),and it have significant difference of the group itself compared before and after treatment(P<0.01).(5) All the blood rheology indexes of the treatment group have statistically significant after treatment(P<0.05 or P<0.01),but without change in the control group (P>0.05);It have statistically significant of the two groups after treatment (P <0.05).(6) Comparison of lung function in the two groups shows that there was no statistically significant difference (P>0.05),but it have statistically significant of the group itself compared before and after treatment(P<0.05).(7) Results of walking distance trial within 6 minutes in the two groups shows that the difference was statistically significant(P< 0.05),and it have statistically significant of the group itself compared before and after treatment(P<0.05 or P< 0.01).(8) Follow-up of acute aggravating circumstances after 3 months’treatment shows that the frequency of acute exacerbations is less than that in the treatment group(P <0.05).Conclusion:Adding Bu Yanghuanwu decoction which takes a part in increasing qi and blood on the basis of conventional western medicine can alleviate the uncomfortable symptoms, improve patients’ quality of life, reduce high blood coagulation state, increase walking distances within 6 minutes and reduce the frequency of acute exacerbations. Moreover, to some extent, it also can delay the deterioration of lung function. Additionally, it is safe without adverse reactions in clinical treatment.
