The Exploration of HM 1A Percutaneous Microwave Coagulator for Tumor Therapy and the Fundamental and Clinical Study on Enlarging the Microwave Coagulation Space (Abstract) 1. Objectives(1) Equipment making : The HM 1A percutaneous microwave coagulation therapy(PMCT) system was self-designed and made, with its function of stability,security and easy operating, to meet the demand of enlarging the microcoagulation volume (MCV)in engineering.(2) Fundamental Experiment:(D To pursue ways of expanding the now limited MCV of PMCT, leading to a effective therapy for hepatic tumor, especially for those larger.(2) To assess to what extend the blood flow influence the MCV.(3) To study the thermal distribution rule of PMCT which is essential to its therapeutic strategy.(D To explicate the relations among the changes of pathology, super-microscopic structure and temperature to the coagulated tissue.(3) Clinical application:(D The self-made micro-coagulator was put into clinical use treating hepatic tumor with great care, evaluating its safety, effectiveness, and adversity primarily.(2) The corresponding relations were disclosed between the micro-coagulation power (MCP), time (MCT) and MCV, MC shape (MCS).2. Materials and methods(1) Equipment making:(D A group of middle and advanced engineers concentrate their advantages to work out system design and system sample(2) We observed the function data of the sample machine systematically:? Long time microwave radiation loading test?The microwave radiation index around the system was measured and adjusted in accordance with the national hygiene standards.?MC tests were implemented in fresh pig liver in vitro, paying special attention to the interconnections between the antenna length and its MCV, MCS to appraise the characters of individual antenna and its possible prospect for clinical use.?On the basis of the pre-stage study, the coagulator system tends to be perfect, through its application on experimental animals(small or big),its security in organism is confirmed, and the relevant data obtained lays a basis for its clinical stage.The configuration of the system was established eventually, ready for the treatment of liver tumours.(2) Fundamental experiment:(D Fresh domestic pig livers in vitro and Newzealand rabbits, domestic pig livers in vivo (with or without blood supply) were coagulated using self-made HM-W1A microwave coagulator.(2) Animal livers with or without blood supply are coagulated in different MCP and MCT in sequence so as to assess the influence of blood flow upon MCV.(3) Experimenting in vitro, we try to describe:The ascending curves of temperature along with the time of the points in the maxium short axis plane.The curves of temperature of the above-mentioned points at certain moments.The temperature characters at the edge of the coagulated area and next to it, the transition area in vitro and in vivo.The influence of the MCP on the curves above.(I) The temperature at the MC core , the edge of the coagulated area and the transition area is studied animal experiment.(D Samples of tissue are prepared for pathology, electron microscope at moments: 0,1,3,5,7,14 days after MC.(3) Clinical application:At first, We used the PMCT system to coagulate a HCC before its surgical resection. In the process, We can observe directly how PMCT interfere with both the life signs and the local tissue.(D PMCT is implemented to treat hepatic tumors under local anesthesia adapting relevant MCP and MCT according to the tumor's size.(D In tumors larger than 5cm in diameter, we coagulated the nutrient vessel area in tumor in priority, and the remnant later adapting the same MCP and MCT, to further illustrate the influence of blood flow on MCV in human body, and the value of the method as therapeutic strategy.(I) The 2-DE, blood flow signals in and around tumors are analyzed before and after the PMCT.3.
