Objective: The purpose of this study was to investigate the effect of radiofrequency catheter ablation (RFCA) on coronary sinus plasma level of endothelin (ET) in patients with reentrant paroxysmal supraventricular tachycardia (PSVT). Materials and mathods:Twenty-two patients ,6 males, ages raging from 15 to 68 years;with PSVT (9 atrial ventricular node reentrant tachycardia (AVNRT), 9 left accessory pathways(LAP) and 4 right accessory pathways (RAP),underwent RFCA successfully. The blood samples were drawn from the coronary sinus before and O,5,1O, 30 minutes after RFCA. The ablation site of pathways, the power of radiofrequency , the times of RFCA and the duration each time were recorded. Plasma ET concentration was measured using radioimmunoassay(RIA).Results:1.The level of ET increased significantly from 46. 36± 10. 92 pg/ml (before RFCA) to peak 69. 11± 16.90 (0 min after RFCA,P=0.0001),and then decreased significantly to 60. 16±12. 93 (at 5 min after RFCA, vs 69. 11± 16. 90,P=0.014),52. 84±6.62(at 10 min after RFCA) and 42.47±9.25( at 30 min after RFCA) (P=0.014 and P=0.0001, compared with 0 min after RFCA, respectively).The ET levels were not significantly different between before and 10 min after RFCA as well as 30 min after RFCA (before vs 10 min after , P=0.072, before vs 30 min after RFCA , P=0.279 respectively) .The level of ETat 5min after RFCA significant differed from the ET concentration before RFCA (P=0.0001) and 10 min after RFCA (P=0.043 ) .2.The ET concentrationofAVNRT (n=9) andAVRT (n=13) had no markly different not only between before and after RFCA every period of time (P >0.05) ,but also the level of ETof LAP(n=9) and RAP(n=4).3.The ET levels of the high 6 energy of rediofrequency(RF) group (> 7000J,n=12) at 0,5,10 min after RFCA were significantly different to the lower energy of RE group (<7000J,n=10) (P<0. 05, respectively) . 4. The level of ET at 0 mm after RFCA was positively correlated with the energy of RF(r=O.496, P=O.O2). Conclusion: The level of ET in coronary sinus plasma transient ascended after RFCA, concerning probably the nectores and injuries of myocardia caused by RFCA.
