| Objective: 1 To study the relationship of type 2 diabetes mellitus and osteoporosis. 2 To investigate into the relationship between estrogen receptor (ER) gene polymorphism and bone mineral density (BMD) of type 2 diabetic patients. Methods: BMD of proximal femur and AP spine in 61 type 2 diabetic patients (T2DM) and 83 normal subjects were measured with dual-energy X-ray absorptiometry (DEXA), and ER gene polymorphism were analyzed by polymerase chain reaction-restriction fragment length polymorphism (PeR- RFLP) in 12DM patients. Results: 1.. The prevalence rate of osteoporosis in type 2 diabetic patients was 44.26%, and the total rate of osteopenia and osteoporosis was 78.69%. The prevalence rate of osteoporosis in male and female patients was 24.00% and 58.33% separately. The total prevalence rate of osteopenia and osteoporosis in male and female patients was 84% and 75% separately. 2 To contrast with young control group, BMD at lumbar spine of 12DM were significantly decreased. Male patients had a tendency of low BMD at Ward's triangle. Female patients had significantly low BMD at Femneck and Ward's triangle. 3 To contrast with age-matched control group, male patients had significantly low BMD at Femneck, Troch, L2, L3, L4 and L2.And female patients had significantly low BMD at Troch and Ward's triangle. 4.. The distribution of ER genotype in 12DM was similar to the normal subjects. The frequency of PP genotype was 0.128, pp genotype 0.539 and Pp genotype 0.333. 5.. 12DM with PP genotype had the lowest BMD especially in spine among three genotype, while patients with Pp or pp genotype had the similar BMD. Conclusion: 1.. 12DM tend to have lower BMD and incur osteoporosis than normal subjects. Male patients were prone to have fracture at lumbar spine for osteoporosis, and to have higher risk of fracture at lumbar spine and hip than age-matched normal male.While female patients tend to have not only hip but also lumbar spine fracture, and to have higher risk of fracture at hip than age- matched normal female. 2.. The frequency of ER genotype was similar to that of normal subjects, that is Pp genotype the most, pp genotype the fewer and PP genotype the fewest. 3.. PP genotype was related to the low BMD of lumbar spine, and æ?allele may be a protect factor of bone mineral capacity. ER genotype can forecast osteoporosis of type 2 diabetic patients. |
