Backgroud: Insulin resistance(IR) or hyperinsulinism(HIS), endothelium damage which can be indicated by the imbalance of endothelin(ET) and nitric oxide(NO) may contribute to the atherogensis and coronary heart disease. Objectives: To study the difference between unstable angina(UA) and stable angina(SA) about JR, ET and NO. To assess the relations of JR and ET, NO with the TCM pattern types in patients with UA. Materials and Methods: The WHO diagnostic criteria of Cl-ID and the TCM pattern types standard were applied. 57 patients with UA (40 male and 17 female, aged 44? 1 years) were studied, including 17 cases of initial onset angina, 30 cases of progressive angina, 10 cases of angina at rest; of TCM pattern types, 18 cases of heart-blood stagnation syndrome, 15 cases of phlegm and turbid stagnation syndrome, and 11 cases with heart-Yin and kidney-Yin deficiency syndrome, 13 cases with the Qi and Yin deficiency syndrome. 28 patients with SA (18 male and 10 female, aged 47?9 years) were observed at the same time. 30 healthy people (20 male and 10 female, aged 45~--70 years) were taken as the control group. The reverse of the product of fasting plasma glucose(FPG) and fasting insulin(FINS) was used as the insulin sensitivity index. The fasting plasma glucose concentration was measured with glucose oxidase method. The fasting insulin and ET concentration were measured with radioimmuoassay. NO concentration was measured with colormeteric method. Results: 1 .There are insulin resistance in both UA and SA compared with control group (P phlegm and turbid stagnation> heart-Yin and kidney-Yin deficiency> Qi and Yin deficiency> control group. 4.The level of ET in TCM pattern types of UA is as follow: heart-blood stagnation>phlegm and turbid stagnation>heart-Yin and kidney-Yin deficiency>Qi and Yin deficiency>control group. 5.The level of NO in TCM pattern types of UA is as follow: heart-Yin and kidney-Yin deficiency, Qi and Yin deficiency
