The P2 integrin CD li/CD 18 which are constitutively expressed on the surface of ieukocytes are heterodimers consisting of a common P subunit (CD 18) and an i subunit (CD 11 a,CD 11 b,CD 11 c) .They mediate cell-cell and cell-matrix contacts by binding their specific ligands allowing firm adhesion, spreading, and subsequent emigration of leukocytes. They play a fundamental role in cell growth,differentiation, inflammation, injury healing. Polymorphonuclear nuetrophil (PMN)adhesion is a major event of the early inflammatory response that is characterized by the recruitment of nuetrophils into areas of inflammation,which begins with the binding of these cells to endothelium cell(EC) followed by their transmigration into tissues.Distinct phases have been characterized during nuetrophils binding, including rolling, activation, and firm adhesion. The molecular basis for these phases is the up-regulation of various cell adhesion molecular that belong to three major families: the selectins, the P2 integrins, and the immunoglobulin superfamily, the selectins are important for rolling, whereas firm adhesion of is primarily P2 integrins-dependent. Mac-i (CD 11 b/CD 18) appears to be the major P2 integrins responsible for the firm adhesion of nuetrophils to endothelium cell and myriad extracellular matrix molecules. Activated-nuetrophils are a double-edged sword, it play important roles in host defense against all classes of infectious agents but, paradoxically, they are also involved in the pathology of various inflammatory conditions. In the primed state, there is no increase in oxidase activity, yet subsequent stimulation provokes a response that is larger than in nonprimed,activated cells.Thus,individual primedcells may be thought of as being "ready to go" but awaiting further stimulus beforethe oxides response is elicited. The primed nuetrophils are thus held at "amber",awaiting "green"before activity is triggered.Recent stUdies have demonstrated that cell,tissue and organ of organism haveamplificational effect on secondary i s chemi a/reperfus io n, metabo l ism o r stre s s o finflammation. Thus, the concept termed as cell priming aPpears,it is refered aspriming that a cell has prepared to first hit. As a first hit, trauma make PMNprimedthat very commonly happens.
