Objects and matbods: 19 stable COPD patients (age:63.41?.75years FEV1.0:0.61?.18L, FVC: 1.28?.31 L) engaged in this trial,after 14-day MPA treatment (20mg tid) and the 2,3,4 weeks after cessation of MPA, all patients were visited and were measured the bodyweight,BP(Systolic BP,Diastolic BP),arterial blood gases(PH,PaCO2,Pa02,HCO3,A-a D02) and flow-volume spirometry.The serum levels of estrogen. testosterone~. progesterone were measured at baseline and the time after 2 weeks of MPA treatment in 10 of 17 patients. The serum Cholestrol and Triglycerides levels were also measured at baseline and at 2 weeks after cessation of MPA of all patients. Results: 17 of 19 patients completed the trial. The mean (盨D) PaCO2 at baseline was 49.89?.2lmmHg and the mean Pa02 was 59.07?.22mniHg.after the 14-day MPA treaiment,the PaCO2 was ~.01?.77 mniHg(p<0.01), the Pa02 was 62.98?.87mxnHg(p<0.0l).We also found a increasing of A-a D02 from 28.3?.46mmHg at the baseline to 34.6?.27mmHg(p<0.01) at the time after the 14-day MPA.there were 12 patients were 揷orrection?the reduction of PaCO2 was over SmrnHg) and 5 patients were 搉o correction?there is no difference between the two groups in bodyweight and flow-volume spirometry at baseline.The changing of the estrogen level was not significant between at the baseline and after the MPA,the reduction of PaCO2 was no correlated with the level of estrogen at the baseline.The average reduction of PaCO2 was 5.7lmmHg(p<0.01) on MPA treatment period and I .25mmHg(p<0.05) after 3 i~eeks cessation of MPA, at 4 weeks after MPA cessation, the PaCO2 returned to baseline.The change of PaO2was 3.48? .6lmmHg(pKO.05) on MPA and 2.02zE4.68mmHg(p<0.05)at 2 weeks after MPA cessation,but there were no significant(1.02?.13) at 3 weeks after MPA cessation.throught the trial, the body m weight, FEV1 .0, FVC, serum Cholesterol level and serum Triglycerides level were not significantly changed.
