AbstractObjective To have a better understanding of lower extremity deep venous thrombosis(LED VT) and summary experience of it.Methods we made a retrospective analysis of 176 patients with LED VT from January 1992 to December 2000 and reviewed some articles.Results 1 DVT is rare in young individuals and becomes more frequent with advancing age. 2 The ratio of left-to-right-sided DVT was1.5:1 .Distal thrombi were more common than proximal thrombi. Postoperative DVTs were often distal, whereas DVT developing in the setting of other diseases was more frequently proximal and often right sided;Proximal,left-sided DVTs were common in the idiopathic subgroup. 3Thrombolytic therapy during early 5 days has a better outcome than that of more than 5 days. 4 Combining thrombolysis with anticoagulation were more effective than using them singly.Conclusions LED VT is caused by several inherited and acquired disorders, sometimes it may also occur spontaneously. The antomic configuration of LEDVT was correlated with etiology;Postoperative DVTs were often distal, whereas DVT developing in the setting of malignancy was more frequently proximal and often right sided;Proximal,left-sided DVTs were common in the idiopathic.Sympotoms and clinical signs are not enough for the diagnosis.We can make a accurate diagnosis by optimal strategies through choosing clinical pretest probability model, Ddimer,ultrasonograpy,venograpy and so on. Thrombolysis and anticoagulation are the major therapy of LED VT. The advent of lowmolecular-weight heparin made anthicogulation more safety and efficacy. The optimum duration of oral anticoagulant treatment is recommended individually. Surgical thrombectomy and vena caval filter should be restricted to a limited number of situations.
