Part l:Left Ventricular Regional Systolic and Diastolic Function in Normal SubjectsObjectives: We measured the regional wall systolic and diastolic indexes on normal subjectives by omni-directional M-mode echocardiography to assess the left ventricular regional wall function and build a control group. Methods: The study group included 40 normal subjects(35 males,5 females), aged 34-71 years old, their short-axis sections at mitral valve level, papillary muscle level and apical level were recorded and analyzed by LEJ-1 omni-directional M-mode echocardiography system developed by radio department of Fujian university. The left ventricle was subdivided into 16 segments of three short-axis sections, wall range during systolic period(R), end-systolic thickness(Ts), end-diastolic thickness(Td), thickness at one-third relaxation period (Tl/3d), systolic velocities(Vs), early-diastolic velocities(Vd) and time from ECG R wave to the beginning of diastole(R-d) of segmental walls were measured, and the percentage of wall thickening( A T%), the Vd/Vs ratio, the percentage of wall thinning in one-third relaxation period(ATl/3d%) ofeach wall were measured. The coefficient of variation of the R-d time(R-D) was calculated to determine the synchronicity of the regional beginning of diastole. Results:The respective average values of all regional functional indexes in total segments were 8.97 ?.5mm(R), 56.47 + 8.2% (AT%), 38.07 + 9.0 cm/s(Vs), 47.67 + 13.9cm/s(Vd), 1.25 ?.17 (Vd/Vs), 64.84 + 6.27% (ATl/3d%). The values of posterior wall were more than those of other walls, the values of interventricular septum were less than those of other walls; the systolic and diastolic motions of basic and middle segments stronger than that of apical ones.Part 2: Studies on Relationship of Quantitative Changes between Regional and Integral Systolic Function of Left Ventricle in Patients with Myocardial InfarctObjectives: To assess the relationship of quantitative changes between regional systolic function and integral systolic function of left ventricle by applying omni-directional M-mode echocardiography. Methods: The study groups included four myocardial infarct(MI) subgroups(group I :EF^55%; group II :50%^EF<55%; groupIII:40% ^EF<50%; group IV :EF<40%) and control group. Their short-axis sections at mitral valve level, papillary muscle level and apical level were recorded and analyzed by LEJ-1 omni-directional M-mode echocardiography system developed by radio department of Fujian university. The left ventricle was subdivided into 16 segments of threeshort-axis sections, wall range during systolic period(R), end-systolic thickness(Ts), end-diastolic thickness(Td) of rengional walls were measured, and the percentage of wall thickening(AT%) of each wall was calculated. Results: The average values of R and AT% of regional walls in each MI subgroup were smaller than those of control group; there were correlations between regional function (average values of R and AT%) and the integral systolic functional indexes(ejective fraction, EF; end-diastlic volume, EDV; end-systolic volume, ESV), and the correlations of AT% were more significant than those of R; in according to the values of AT%, the area of involved region of each MI subgroup increased( I group:10.63%, 1.7 segments; II group: 14.38%, 2.3 segments; III group:26.25%, 4.2 segments; IV group:53.75%, 8.6 segments).Part 3: The New Regional Diastolic Functional Indexes and The Relationship of Quantitative Changes between Regional and Integral Diastolic Function of Left Ventricle in Patients with HypertensionObjectives: To study the relationship of quantitative changes between regional diastolic function and integral diastolic function by omni-directional M-mode echocardiography, meanwhile assessed the practicality of Vd/Vs and ATl/3d%.Methods: The study groups included hypertension group(diastolicdysfunction group) and control group(normal diastolic function group), the hypertension group was divided into two subgroups in according to th...
