Systemic inflammatory response syndrome (SIRS) was a conception first brought forward in America in 1991 referring to the uncontrolled systemic inflammatory cascade induced by infectious or noninfectious factors. It always leads to pyaemic shock and multiple organ dysfunction syndrome. Nowadays there is no effective therapeutics, and treatment by Chinese herbal medicine is emphasizede more and more.Endotoxin plays a pivotal role in development of SIRS. The lung is the vulnuerable organ to SIRS. So the sufferor should suffer from pulmonary infection infected by G" bacilli during SIRS. The aim of this study was to investigate the clinical curative effect of Chuanhuning injector on pneumonia during SIRS and the probably mechanisms.We have set up two groups: observing bundle and control bundle. Each bundle has 30 patients. Observing bundle treats with Chanhuning and Lailixin. Control bundle treats with Lail ixin. Studies includes: symptom, phys ical sign,blood routine (such as white blood cell differential count,ect) and serum levels of TNF-a and LPS. The result is observing bundle is better than control bundle at clinical curative effect.In order to investigate the probable mechanism, Chuanhuning and Lailixin's effect on the production of TNF-a by alveolar macrophage (AM) was studied. Using cell culture of AM from normal rat, we observed: the levels of TNF-a in culture supernatants (ELISA), the expression of I-kB in AM(immunocytochemistry), the levels of LPS in culture supernatant(quantitative Limus Assay) and the effect on the structure of LPS (observe by electron microscope). The result shows: Chanhuning could suppress the production of TNF-a and the low-expression of I-kB induced by LPS, destruct the normal structure, reduce the levels of LPS in culture supernatant.In this study, we demonstrated that Chanhuning injector could prevent and cure the pneumonia during SIRS in earlier period. The mechanism was probably that Chanhuning could restrain the production of TNF-a through two pathways: ?destructing the structure of LPS directly, ﹕uppressing the low expression of I-kB in the nucleus of AM.
