Objective: The incidence of obesity and diabetes mellitus are going up gradually these years. Insulin resistance is generally considered as a major pathophysiological link between these two disorders. Studying on the forming mechanism of obesity-associated insulin resistance is a hotspot nowadays. This study are mainly about: (1) The expression level of tumor necrosis factor-a in the adipose tissue of obesity group and control group. (2) The expression level of tumor necrosis factor-a in serum of obese and non-obese type 2 diabetes mellitus and control. (3) Free fatty acid concentration in serum of obese and non-obese type 2 diabetes mellitus and control.Method: (1) People of body mass index(BMI) >25kg/m2 and BMI<25 kg/m2 were divided into experiment group and control group respectively. The expression of TNF-a in adipose tissue was tested with enzyme-linked immune sorbent assay(ELISA). (2) Patients of obese and non-obese type 2 diabetes mellitus were divided into two experiment groups respectively, and health people were taken as control group. The expression level of TNF-a in serum was tested with ELISA. (3) Patients of obese and non-obese type 2 diabetes mellitus were divided into two experiment groups respectively, and healthpeople were taken as control group. The quantity of free fatty acid in serum was tested by high performance liquid chromatagraphy (HPLC).Result: (1) TNF-a level of adipose tissue in obesity group is higher than that in control group, and there's a strong positive correlation between the expression of TNF-a and BMI. (2) The expression level of TNF-a in serum of obese type 2 diabetes mellitus is higher than that of non-obese type 2 diabetes mellitus and control people, and it has positive correlation with BMK TG and negative correlation with insulin sensitivity index(ISI). (3) The quantity of free fatty acid in serum of obese type 2 diabetes mellitus is higher than that of non-obese type 2 mellitus and control people, and it has positive correlation with glucose ^ BML TG, and negative correlation with insulin sensitivity index(ISI). Saturated/unsaturated fatty acid ratio is no difference between obesity and non-obesity group, both of them are higher than that of control group.Conclusion: (1) TNF-a is related to obesity, and expresses highly in adipose tissue of obesity. (2) Obesity group of type 2 diabetes mellitus have higher TNF-a level. Possibility TNF-a has important effect on insulin resistant. (3) Obesity group of type 2 diabetesmellitus have higher free fatty acid level. Possibility free fatty acid hasimportant effect on insulin resistant, and different FFA has differenteffect.
