Objective: To explore the association of essential hypertension(EH) with ACE gene polymorphisms, insulin resistance(IR) and type 1 plasminogen activator inhibitor (PAI-1) in Qingdao old population. Method: Choose 107 cases essential hypertension and the 96 cases of normal blood pressure patients hospitalized in Qingdao municipal hospital. Their fasting blood sugar, fasting blood insulin, C-peptide, PAI-1 were measured. ACE gene polymorphisms w:ere measured by using PCR method. Result: (1) There were significant difference of PAI-1, insulin, C-peptide and Insulin Sensitivity Index (ISI) between hypertension patient and normal blood pressure group (P<0.01). (2) The I/D polymorphism in intron 16 of the ACE gene was categorized into three genotypes: two deletion alleles (genotype DD), heterozygous alleles (genotype ID) and two insertion alleles (genotype II). There was no significant different in genotype pattern of DD, ID. II and the frequency of I, D allele betweenessential hypertension group and normal blood pressure group (P>0.05). (3) The ISI was significant different between II gene type and DD gene type patients In EH group (P<0.05), and the difference of PAI-1 between DD gene type and II gene type patients in EH group was significant (P<0.05). Conclusion: There was insulin resistance in essential hypertension patients. The I/D polymorphism of the ACE gene was not associated with essential hypertension, but may be a predictive factor for insulin resistance. It may easier cause insulin resistance with ACE II genotype. There was association between essential hypertension and increase of PAI-1. PAI-1 may was higher in essential hypertension patients with DD genotype.
