Objective:To explore the active component of Astragalus membranaceus & Angelica sinensis (A & A ) treating nephrotic syndrome(NS) and its pharmacokinetic characters. From the points of cell factors and the chemical component from the compound herbal prescription, we try to probe the mechanism of A& A treating NS and provide the pharmacokinetic evidence for the clinic treatment.Methods:A new method for the determination of ferulic acid (FA) in serum samples by HPLC has developed. Serum samples containing ferulic acid and internal standard coumarin (IS) were firstly treated in boiling water-bath and then methanol to deproteinate. And the FA concentration in serum of healthy volunteers and patients with nephrotic syndrome were determined after oral administration of A&A decoction, and then their pharmacokinetic parameters were analysed by 3p97 software. We also detected the ET and NO in serum of above healthy volunteers and patients, and found the effect of A & A on the ET and NO in the patients with NS.Results:The absorption of FA in the patients with NS is lower, and the half-life is longer than that in the healthy volunteers. The main pharmacokinetic parameters as follows: Ka (min'1) : 0.2530 ?0. 0349 and 0.2426 ?0. 0236 ; T1/2a(min): 43.7899 ?5. 5863 and 91.5569 ?8. 2949 ;Tp( min): 18.0167 ?5. 8823 and 24.9890 ?2. 9823; Cmax( u g/ml) : 159.5212 ?13.9936 and 130.2247 ?10.9846; T1/2, (rain): 291.2811 ?4. 5753 and 418. 1090 ?19. 5201; AUC( u g ?rain/ml): 60888.3320 ?2153. 9644 and 61496. 6370 ?6471. 3452; V/F( c )(L/kg): 0.0066 ?0. 0022 and 0.0074 ?0. 0018. The concentrations of ET and NO in the serum of healthyvolunteers are respectively 43.95 ?8.46 pg/ml and 421.01 ?25.31 pmol/L. The concentrations in patients with NS are respectively 135.45 ?10. 37 pg/ml and 279.34 + 23. 75 pmol/L, after given A&A, the ET is decreased (98.56 ?11.45 pg/ml) and the NO is increased (356.25 ?2. 64 pmol/L). Conclutions:? Serum samples, treated by boiling water bath and methanol, were determined directly by high performance liquid chromotogrphy(HPLC),which is favor of prelecting chromatogram column and enhancing recovery rate and being more repeatable.?The absorption of FA in the patients with NS and Asthenia of kidney is lower, and the half-life is longer than that in the healthy volunteers.? Aft A can decrease the level of ET and increase the level of NO in serum of the patients with NS. FA is an active component of A & A .? Pharmacokinetic characters of FA are distinctly related to the effect of A&A, and therapeutic drug monitoring should be carried out carefully in patients with NS taking Chinese medicines.
