The Research on Gene Expression ofRelated Cytokines In Rat CombinedLiver/Small Bowel TransplantationCombined liver/small bowel transplantation is not only the therapy demands of short bowel syndrome with liver failure, but also an effect way to resolve rejection of small bowel transplantation by immuno-privileged liver in combined transplantation including small bowel. This study was to investigate the features of cytokines in liver/small bowel transplantation (LSBTx) and study mechanism of immunologic tolerance following organs transplantation.In the rat strain of Wistar桽D, grafts were tolerated in orthotopic liver transplantation, but were rejected in small bowel transplantation without immunosuppression. According to this phenomenon, Liver transplantation(LTx^ small bowel transplantation (SBTx) and LSBTx(Wistar-^SD) were performed and the experimental models were divided into three groups: group 1. LSBTx (n=18); group 2. SBTx (n=12); group 3. LTx (n=12). Liver grafts were harvested in LSBTx and LTx on POD7,14. Small bowel grafts were harvested in SBTx on PODS, 7 and in LSBTx on PODS, 7, 14. Survival, pathology and cytokine including TL-2, IL-4, IFN- Y were investigated by means of sequential clinic observation, histology and reverse transcription polymerase chain reaction(RT-PCR).Results. All the recipients of LTx and LSBTx survived more than 28 days, but the SBTx recipients were rejected and died within 19 days. The histologicalfeatures showed that rejection was found in all grafts and more severe rejection presented in the small bowel of SBTx than in that of LSBTx on POD 5 and 7. On POD 14, the rejection of small bowel in LSBTx have released obviously. Similar rejection occurred on POD 7 and 14 in liver between LSBTx and LTx. The EL-2 coding gene expression had no significant difference in all comparisons between LSBTx and LTx or SBTx within 14 days. On POD 5,transcription of IL-4 mRNA in SBTx keep at a considerably higher level than that in the small bowel of LSBTx. The liver EFN- y mRNA expression of LSBTx was significantly higher than that of LTx on POD 14 The level of small bowel EFN- Y was significantly higher in SBTx than in LSBTx on POD 5 and 7.Conclusion. 1. Liver successfully induced tolerance of small bowel in rat LSBTx. 2. The tolerance in liver transplantation is the process related to immune activation. 3. The level of EL-2 in grafts is independent of finally destroy of grafts in early stage after transplantation, though EL-2 is an important factor for the beginning of rejection. 4. EL-4 participated in acute rejection process likewise. The decline of EL-4 in early stage might be connected with tolerance. 5 The expression of EFN- y coding gene in graft is an important prognosis factor to evaluate and judge intensity of rejection. Persistently lower level of EFN- y in grafts in early stage after transplantation play an important role in the tolerance of small bowel induced by liver.
