Psoriasis is a complex disease with diverse clinical presentations and with polygenic inheritance , increasing evidence indicates that T lymphocyte activation is the central feature of the pathogenesis of psoriasis. Today the mechanisms of lymphocyte activation in psoriasis is not clear. For the scarcity of studies on ultrastructure of intralesional immunocyte , the purpose of this subject is to study the pathogenesis of psoriasis by observing the ultrastruture of in intralesional lymphocyte , macrophage with the election microscopy. Thirty-five vulgaris psoriasis cases were selected in comparison with twenty normal people. We biopsied the lesions before treatment. After fixing, embedding , polymerizing, staining, we observed them by electron microscopy. All the diagnoses were proved by clinical and histological examination. We found that there was a phenomenon of a contact between lymphocyte and macrophage in the dermis of the vulgaris psoriasis. It appeared an intimate contact of lymphocyte-lymphocyte(28/35); lymphocyte-macrophage(23/35). But there was no such a phenomenon in the all compared cases. As the two cells contacted, it showed an alteration of the cellular form and the contacting cells showed a phenomenon of functional activity such as the volume of the cell increases, the color of the nucleus is darker, etc. That suggested the contact of the cells was not simply a contact of anatomical level Such close relationship probably showed that there was a direct interaction among these cells. That, to a certain extent, accords with the studies on the pathogenesis of psoriasis presently in China and abroad. The intimate contact of the intralesional cells of psoriasis probably plays a certain role in the initiation and maintenance ofpsoriasis. Moreover our discovery provides a new way for the study on the pathogenesis of psoriasis. With further study, we may enable research workers to have more knowledge on the pathogenesis of psoriasis.
