Objective To assess whether urokinase administered into the ventricles with low ventricular drainage can improve the prognosis of primary intraventricular hemorrhage in adults. Materials and Methods Forty-two cases of primary intraventricular hemorrhage in adults in our department from March 1994 to December 2001 were treated with intraventricular infusion of urokinase in low ventricular drainage, we analysed the clinical features, risk factors, feasibility and effectiveness of urokinase, mechnism of low ventricular drainage,and outcome. In our group,the most frequent complaint was headache in all patients, followed by nausea/ vomiting in 34 patients, seizure in 6 patients. On examination, 31 patients had altered mental status, 6 patients had pupillary abnormalities, 5 patients had motor deficits.The associated risk factors were hypertension in 20 cases, arteriovenous malformation in 7 cases, aneurysm in 2 cases, moyamoya disease in 1 case and carotid occlusion in 1 case. Receiving urokinase did not increase the occurrence of hemorrhage complications or of infectious ventriculitis.Result Twenty-six patients had a good outcome,moderate disability in 4 patients,severe disability in 2 patients, vegetative state in 2 patients, death in 8 patients, mortality was 19.1%.Conclusion We conclude that urokinase administered into the ventricle with low ventricular drainage is effective in the treatment of primary intraventricular hemorrhage in adults, and this relatively easy method can improve the prognosis of primary intraventricular hemorrhage.Graduate Xiu Chun Ming(Neurosurgery) Directed by Meng Qing Hai...
