Discuss About The Clinical Audiology Character Of Kidney Deficiency And Live-fire Of Tinnitus And Deafness

Objective: Tinnitus and deafness which is a kind of commonly encountered disease in otorhinolaryngology , whose clinical treatment now is generally accepeted one of the problems in the world. The traditional Chinese medicine (TCM) has great effect on the problem of prevent ion and cure of which has been generally accepted in the world more and more. The superiority and soul of TCM are diagnosis and treatment, so are the tinnitus and deafness. However, many patients suffer only tinnitus and\or deafness and have no other symptom which have great difficulties in differentiating syndromes by traditional means. The present study in which we adopt the authoritative criteria of differentiation of syndromes and make full use of the modern advanced audiographies, combine the statistical method of multivariate discriminant analysis to discuss about the clinical audiology of the two best familiar forms-kidney deficiency and liver-fire. What more, in order to set up the objective diagnosis marker and solve the problems of diagnosis, and improve the veracity of TCM diagnosis and bring into full play in effect on tinnitus and deafness -one of the problems in the world.Design: a hospital-based cross-sectional studySetting: Otorhinolaryngology Department of First affiliated Hospital of Guangzhou University of Traditional Chinese MedicinePatients\participants: All the patients involved in the study were the outpatients or inpatients of the Otorhinolaryngology . Department of First affiliated Hospital of Guangzhou University of Traditional Chinese Medicine. The selected cases were those meeting the inclusive criteria as follows: The diagnosis must coincide to kidney deficiency or liver-fire of TCM; The case must be above seven old years; and the case can fit all of the examinations. Elimination criteria: The case which belong to tinnitus and deafness due to the diseases of external ear or middle ear or center lesion. Except the capital symptoms tinnitus and deafness the case which have neither other signs nor the typical diagnosis of TCM; The cases must be Eliminate no matter how not to have the systemic hearing tests( including the cases whose hearing loss>90dB).Main Outcome Measures: The two associate professors or professors take charge of differentiation of syndromes by establishing uniform criteria and questionnaire. The cases including 30 normal persons as comparison which must be given pure-tone test and acoustic immittance test and ABR test. All of examination of results should be establish multivariate discriminant functions of the kidney deficiency and live-fire of tinnitus and deafness according to the statistical methods of ANOVA and x2test and multivariate discriminant, and the results will be verified.Conclusions:1 .Kidney deficiency and live-fire of tinnitus and deafness have hearing loss in different audio frequencies, whose tinnitus frequencies and the scores of SISI test are very high and both sides of auditory reflex threshold have risen in some degree and the surplus between the auditory reflex threshold and pure-tone threshold degrease in different audio frequencies, which the latent period of Dwave and Dwave of ABR extend and the click hearing and reaction threshold have increased in some degree 1kHz and 4kHz.2.The group of kidney deficiency have higher hearing threshold for speech and conscious loudness index than the group of live-fire. Many of acoustic curves of the group of kidney deficiency are curves of slow drop, but most of the other group are the curves of sudden drop; The subtract between the acoustic reflex threshold of 1kHz or 2kHz and the pure-tone hearing threshold which the group of kidney deficiency has lower than the others. As to the latent period of I wave in ABR test, the group of kidney deficiency has longer than the others.
