OBJECTIVE: To observe the relationship between pro-inflammtory cytokine (TNF a , IL-1 ? and IL-6) and acute lung injury.METHOD: The study is a foresight investigated. According to APACHE II, the patients have been divided into two groups: critical group(APACHE II?16) and normal group (APACHE II<16). Pro-inflammatory cytokine (Tumor Necrosis Factor ? , interlukine-1? and interlukine-6) was tested by ELISA in 24 hours and on 48 hours , 120 hours after admission and observe the relationship between acute lung injury and APACHE II, secretion of pro-inflammation cytokines and mortality.RESULT: Among 102 cases, 52 cases have ALI, morbidity is 51%. 32 cases died, mortality is 33% . Among ALI patients, mortality is 42%(22/52). Compare critical group with general group, the critical group has higher ALI morbidity (65%:43%, p<0.05) and higher mortality(2 1.7%: 16.0%, p<0.01). Generating of ALI have the positive relation with APACHE II and great secretion of pro-inflammatory cytokine in the early stage, special for IL-1?. The mortality had the positive relation with secretion of pro-inflammatory cytokine in the early stage.CONCLUSION: The great secretion of pro-inflammatory cytokine in early stage may induce ALL...
