Background Shortage of the liver is one of the main reasons hindering the development of liver transplantation. Partial orthotopic liver transplantation(POLT) is an effective method to solve the problem. Living donor liver transplantation has been developed in 1980s abroad. In the field, some research has been done in recent years. Study on ischemia/reperfusion after POLT has been done a little and it need to do much more.Object The purpose of this article was to establish a method which can predict the partial liver volume of SD mouse according to liver computed tomography (CT) scan, which will help us to learn something about small graft in partial orthotopic liver transplantation(POLT). In addition, a mouse model of partial liver transplantation was built and the difference of apoptosis induced byischemia/reperfusion between POLT and orthotopic liver transplantation(OLT) was compared. In the meantime, influence of splenectomy on them was observed.Method Spiral CT (plain and enhanced) was performed on 30 mice to measure the liver volume and the body weight of rats and liver weight were also measured. The relationship between liver weight and liver volume, body weight, body surface area or CT calculated volume was explored by linear regression. One hundred and eight SD mice (250-300g) were randomly divided into three groups: orthotopic liver transplantation group(group A, ri=36), partial orthotopic liver transplantation (group B, n=36), partial orthotopic liver transplantation combined with splenectomy(group C, n=36). The effect of apoptosis and expression of its related gene (Bax) were studied by TUNEL method and immimohistochemistry(SP method), respectively. The morphological changes were investigated by electron microscopy.Result The experiment shows that value of mouse liver weight (11.14+1.55)g is almost equal to that of liver volume (11.20 + 1.51)cm3 and the ratio of lobe in the whole liver is constant. The relationship between the weight of the liver and body weight, body surface and the calculated CT volume was linear (P<0.01). Moreover, the CT technique was found to be more accurate than others. Besides this, the hepatocellular apoptosis was more obvious in POLT group than in OLT group, after splenectomy, the numberof apoptosic hepatocytes in group C decressed significantly compared with that of POLT group(P<0.05). It maybe relate with inflow of portal vein. And the Bax gene expression was accordance with this finding. The morphological changes of hepatocyte and sinusoidal endothelial cells were reduced by splenectomy in group C.Conclusion The liver volume can be predicted through rat body weight, body surface area, and CT scan. CT scan can evaluate the liver volume of every lobe, which can be used in the research of the smallest graft. The apoptosis was more obvious in POLT than in OLT , splenectomy can reduce apoptosis of hepatocytes in POLT. It maybe relate with inflow of portal vein and this function maybe connected with the expression of apoptosis inducing gene Bax.
