The Clinical Importance Of L-arginine Supplemented PN In The Patients With Colorectal Cancer

Objectives:To study the effects of L-arginine supplemented PN on the immufunction - protein metabolism and complications in the patients with colorectal cancer after operation,and achive the objective of reinforcing immunological function and improving protein metabolism and decreasing the incidence of complications in patients with colorectal cancer after operation,thus promote the effect of parenteral nutrition. Methods:Forty patients received a either a standard PN or PN supplemented with 20g/d L-arginine for 7 days after colorectal cancer operation . Amimlative whole blood from 40 patients with colorectal cancer ,ldays,4days and 7days after operation was sampled. Wedected CD3+ CD4+ CD8+ NK,ratio of CD4+/CD8+from peripheral blood with immunofluorescence by using multicolor flowcytometry,andC3 C4 CH50 IgG IgA IgM by transmission turbidmetry,and nitrogen balance et other marks.We compare the incidence of complication between two groups.All data were statistical analysised with Ssps 10.0. Results: The patients with colorectal cancer had immune suppression beforeoperation,representing that the percentages of CD3+ CD4+ CD25 C4CH50 were obviously lower(P<0.05).(2) The percentages of CD4+ CD4+ /CD8+ NK,IL-2R obviously rose in 4days and 7days after operation in study group,The percentages ofCD4+ > CD4+/CD8+ IL-2R exceeded the preoperative level,and there were significantly difference between two groups (P<0.05).(3) IgA IgG IgM C4 CH50 obviously rose in 4days and 7days after operation in study group,C4 CH50 exceeded the preoperative level,and there were significantly difference in C4 between two groups (P<0.05).In the study group the nitrogen balance become positive from the 4th day after operation,while in the control group the nitrogen balance still remains negative.(5) There were no complications in study group after operation,but two nick complications in control group.and there were no complications of parenteral nutrition.Conclusions:Arginine can improve the immufuntion and protein metabolismin the patients with colorectal cancer after operation,decrease the incidenceofcomplications and use safetly,and enhence the effect of parental nutritioneffectively.
