Objective: To explore a posterior decompressive surgery(PDS) of treating central lumbar spinal canal stenosis(CLSCS), observe clinical effect and discuss some related problem. Method: To adopt completely decompressive surgery after total laminectomy and reconstruct the complex structure of spinous process and ligament in original place through the central incision of spinal posterior approach, at the same time, to keep the integrality of the complex structure of spinous process and ligament. Result: We get a preferable curative effective with the completely decompressive surgery of keeping the complex structure of spinous process and ligament in treating central lumbar spinal canal stenosis. Conclusion: It is commonly and widely used that the decompressive surgery treats lumbar spinal canal stenosis through the central incision of spinal posterior approach. The complex structure of spinous process and ligament is completely ablated and the posterior column structure is severely destroyed with classical PDS treating central lumbar spinal canal stenosis. And this can result in the postoperative spinal instability and reduce the future clinical effect. The completely decompressive surgery of keeping the complex structure of spinous process and ligament can completely decompress and assure the spinal stability to a certain extent. Being short of observation case, the future curative effect need the further observation.
