Objective Angiotensin II (AngII) and aldosterone (ALD) usually increase when ACEI is used in EH for a long term .It means that ACEI therapy induces aldosterone escape in vessels and hearts in EH. Lotensin is a kind of ACEI with a carboryl.Valsartan is a kind of ARE .This test is to observe whether there is an aldosterone escape in ACEI or ARB when they are used in EH by comparative effects on RAS and ET . Methods: The sixty cases according to WHO were divided into two groups ramdonly and each each group has thirty cases. Group 1 was treated with lotensin lomg qd for two weeks .Group 2 was treated with valsartan 80mg qd for two weeks.All the cases excepted from these diseases,such as secondary hypertension, acute myocardval infarction in recent three months,uncontrolled diabetes and serious arrhythmias .All the patients didn't take antihypertensive medicine for 2 weeks out hospital. And they were from 30 to 78 years old.Plasma rennin activity (PRA) and Ang II and ALD and endothelin(ET) and systolic blood pressure(SBP) and diastolic blood pressure(DSP) were measured before and after the treatments Resultes:! The comparation of lotensin and valsartanbefore and after the treatments: ET and SBP and DBP decreased in group 1 after the treatments with significant difference.There wasn't significant difference inPRA and Ang II and ALD in group 1 after the treatments . Ang II and ALD and ET and SBP and DBP decreased in group 2 after the treatments with significant difference. PRA showed no changes in group 2 after the treatments. (2) The comparation of lotensin and valsartan. There was no difference between the two groups before the treatments .After the treatments,PRA and Ang II and ALD and SBP and DBP decreased in group 2 with significant difference. Conclusion:Long-term lotensin induces aldosterone escape but varlsartan has better antihypertension function on EH and no aldosteron escape .Both of them can improve endothehum function .But there is no difference between lotensin and varlsartan on EH .
