Objective To analyze the association of transforming growth factor-β1(TGF-β1) gene+869 polymorphism and the serum level of TGF-β1 in patients with early essentialhypertension and early renal injury. To study the effect of the gene polymorphism ofTGF-β1 on the individual treatment of valsartan.Methods 80 patients with early essential hypertension and early renal injury were treatedwith valsartan, their polymorphism of TGF-β1 gene +869T/C were analyzed bysequence specific primers-polymerase chain reaction (PCR-SSP) methods. Before givenvalsartan and after the treatment of 8 weeks, their urinary ALB were determined, theirserum levels of TGF-β1 were determined by enzyme-linked immunosorbent assay(ELISA).Results The blood pressure level of TGF-β1 gene +869T/C CC gene type, of TT genetype, of gene TC type showed no statistical significant difference (P>0.05). The urinaryAlb level of the three gene types was: CC gene type's>TC gene type's>TT gene type's,the urinary Alb level of CC gene type was significantly higher than the level of TC genetype and of TT gene type (P<0.01). The serum level of TGF-β1 in the three gene typeswas: CC gene type's>TC gene type's>TT gene type's. There was statisticalsignificant positive correlation between the urinary Alb level and the serum level ofTGF-β1(P<0.05). After 8 weeks treatment with valsartan, there was significantdifference in the reduction of the blood pressure, in the reduction of the urinary Alb level, in the reduction of the serum level of TGF-β1. However, the blood pressure reduction ofthe three types has no statistical significant difference. The levels of urinary Alb reductionwas: CC gene type's>TC gene type's>TT gene type's, the reduction level of urinaryALB in CC gene type was higher than the reduction level of TC gene type and of TT genetype (P<0.05). But the reduction percentage of the urinary Alb showed no statisticalsignificant difference (P>0.05). The reduction of serum level of TGF-β1 in the threegene types was: CC gene type's>TC gene type's>TT gene type's, there was highlysignificant difference between each other (P<0.01). However, the reduction percentageof the serum level of TGF-β1 in the three type showed no statistical significant difference(P>0.05).Conclusion 1. The renal injury of CC gene type in patients with early essentialhypertension and early renal injury was more serious than the injury of TC gene type andthe injury of TT gene type. 2. The serum level of TGF-β1 can accurately indicate theseverity of the renal injury and the protective effect on kidney with valsartan in patientswith essential hypertension and early renal injury. 3. The treatment effect of valsartan inthe patients of different TGF-β1+869T/C gene type has no difference, it is saidthat, while effectively decreasing the blood pressure and protecting the renal function,valsartan can reduce the serum level of TGF-β1 without being limited by the genepolymorphism of TGF-β1.
