Objective:To investigate the relationship between classification about verticalunstable pelvic fractures and open reduction and internal fixation in order toprovide the basis for the clinic internal fixation choice and to rise the healingrate of vertical unstable pelvic fracture.Methods:Eighty-four patients with vertical unstable pelvic fractures havebeen treated by open reduction and internal fixation in eight years. Amongthem: 13 cases is type I ;25 cases type II; 5 cases type III; 13 cases type IV; 20cases typeV; 8 cases type II+IV .The fractures of type I, II, III have beenfixated by T-shaped plate or pelvic-reconstructed plate through anteriorapproach. The fractures of type III, IV, V have been treated byTr-shaperod ,sacral rod or cancellotis hollow screw through posterior approach. 18cases of anterior pelvic ring fractures have been fixed by superior ramus ofpubis mtramedullary hollow screw or pelvic-reconstructed plate.Results:The average range of follow-up is 18 months .The union rate is 100%.All patient's lower limbs are equal. The pelvic abnormity have been rectified.Healing rate is 92.9%.Conclusion:The choice of the proper operative approach and reasonableinternal fixation to treat vertical unstable pelvic fractures by allowing forfracture classification and patient's specific condition can bring aboutsatisfactory results. The anterior pelvic fractures should be fixed as far aspossible.
