Objective To search into the distributive regularity of ACE genetype in hypertensives and kidney lesion , and to explore the relationship between angiotensin converting enzyme gene polymorphism and hypertensive kidney lesion and PAI-1 in hypertensives, so that to reveal the molecular mechanism of this disease. Methods ACE genotyping with polymorase chain reaction (PCR) was performed in 137 hypertensives (including 67 hypertensives without kidney and 70 hypertensives complicated with kidney lesion )and 96 unrelated healthy controls;The plasma PAI-1 was determined with ELISA;ACE activity was determined by ultraviolet method; Ang II , B2~MG and uric albumin were measured by radioimmunoassay. Results (1)The frequencies of DD genotype and deletion allele among hypertensives complicated with kidney lesion were higher than those among healthy controls, x 2 were 6.8589,5.6162, respectively (P<0. 05) . The frequencies of DD genotype and deletion allele among hypertensives without kidney lesion campared with healthy controls haven't show significant differences, x2 were 0.0398, 0.0235 respectively (P>0. 05) . (2)There were significant differences of ACE, Ang II , PAI-1 between DD genotype , ID genotype and II genotype among hypertentives (P<0. 05) . (3)The plasma Angll correlated with ACE and PAI-1 (r were 0. 4125, 0. 4807 respectively) . (4)No significant differences could be detected between ACE gene I/D polymorphism and hypertensives. Conclusions (1)The I/D polymorphism of ACE gene is not associated with essential hypertension, but DD genotype of ACE gene may be a risk forhypertensive kidney lesion. (2)The plasma ACE, Anglland PAI-l level was associated with ACE gene polymorphism. (3)There was ECM deposition in hypertensive kidney lesion. ACE gene polymorphism may play an important role in ECM deposition.
