An Experimental Study On DSA Lingual-arterial Infusion Embolization With Carboplatin And 5-Fu In Dogs

Objective: To measure the concentration change of anticancer drugs in plasma and tissue after transfemoral superselective intra lingual arterial infusion embolization under DSA and intravenous injection; and to evaluate the change of liver and renal function; and to determine the histopathologic effect of lingual arterial by different toutes of administration.Methods: Four of five dogs selected were divided randomly into group 1 (n=2) and group 2(n=2). In group 1, 5-fluorouracil(5-Fu) 75mg and carboplatin(CBP) 75mg were injected firstly into dogs via vein, then transfemoral superselective intra lingual arterial infusion gelfoam embolization under DSA (digital subtraction angiography ) was done with 5-fluorouracil(5-Fu) 75mg and carboplatin(CBP) 75mg after 3 weeks. Vein blood sampies at 10min, 20min, 30min, lh,2h, 3h, 5h,3d,5d,8d time points, and the lingual tissue and the buccal tissue specimens at lOmin, 20min, 30min, 3d,5d,8d time points were collected after drugs administration each time. Drug concentrations in plasma and tissue were determined by by high-performance liquid chromatography (HPLC). Meanwhile, at Od,3d,5d,8d vein blood was taken to evaluate the change of liver and renal function. In group 2, the diference was that drugs were delivered firstly via intra artery, then via vein. The biopsy of the infusion lingual arteries and the noninfusion arteries was done to determine the histopathologic effect of lingual arterial by different routes of administration.Results: (1)The attenuation of drugs concentration in plasma after intraarterial infusion embolization was lower than after injection via vein. At lOmin, 20min, 30min after intraarterial infusion embolization, the 5-fluorouracil concentration in tissue of the tongue was shown to be 25(P<0.01),20(P<0.01) and 4( P<0.05) fold high than after injection via vein, and to be 25(P<0.01),44(P<0.01) and 25(P<0.05) fold high than after injection via vein in buccal tissue. (2)Almost no differences of the change of liver and renal function and no ditterences of the change of WBC, GRAN, RBC, HGB, MCV, PLC were shown between after drugs administration via arterial and vein.(3)No consistent histological ditterences were detected between injection and noninjection lingual arteries. Both demonstrated smooth muscle cell vacuolation, and smaller fraction endometrium abscission compearing with no drugsadministration into lingual artery. To some extent the above change demonstrated obviously in injection arteries.Conclusions: Intraarterial infusion embolization with carboplatin and 5-Fu is a better ways in treating oral and maxillofacial malignant tumor.These injection vessels can be used with caution as recipient vessels for FTT (free tssue transfer)in patient undergoing surgical salvage.Postgratuate: Wang Liangzhong (Oral and Maxillofacial Surgery) Directed by: Associate Professor Fan GongweiProfessor Li Ningyi...
