PART I THE EFFECT OF PREOPERATIVE INTRAARTERIAL INFUSION CHEMOTHERAPY ON THE EXPRESSION OF SMAD2 AND SMAD4 IN GASTROINTESTINAL CARCINOMAObjective: To detect the expression of Smad2 and Smad4 in gastrointestinal carcinoma after preoperative intraarterial infusion chemotherapy and to discuss the effect of preoperative intraarterial infusion chemotherapy on the Smad pathway in gastrointestinal tract .Methods: 27 cases with advanced gastrointestinal Carcinoma were treated by preoperative intraarterial infusion chemotherapy while as 27 cases by venous chemotherapy and 36 cases by operation served as the control group. The expression of Smad2 and Smad4 was detected by immunohistochemic S-P method. Result: In gastric carcinoma, compared with the venous chemotherapy group and the preoperative intraarterial infusion chemotherapy group ,the expression of Smad4 in operation served group was significantly lower(p<0.05),compared with the venous chemotherapy group, the expression of Smad4 in the preoperative intraarterial infusion chemotherapy group was not significantly higher(p>0.05).In colorectal carcinoma, the expression of Smad4 was similar with in gastric carcinoma . After by chemotherapy, the expression of Smad2 was not significantly higher in gastric and colorectal carcinoma(p>0.05).Conclusion: Chemotherapy affected Smad4 directly but not Smad2 and renewed Smad pathway in gastrointestinal Carcinoma to treat it. Preoperative intraarterial infusion chemotherapy could be an effective chemotherapy method to treatment advanced gastrointestinal Carcinoma similar with vein chemotherapy. PART II THE EFFECT OF PREOPERATIVE INTRAARTERIAL INFUSION CHEMOTHERAPY ON THE CELL APOPTOSIS AND PROLIFERATION IN GASTROINTESTINAL CARCINOMA AND ITS CORRELATION WITH SMAD PATHWAYObjective To detect the transmutation of PCNA and AI in gastrointestinal carcinoma after preoperative intraarterial infusion chemotherapy and to discuss the efect of preoperative intraarterial infusion chemotherapy on the cell apoptosis and proliferation in gastrointestinal carcinoma and its correlation with Smad pathway.Methods 27 cases with advanced gastrointestinal Carcinoma were treated by preoperative intraarterial infusion chemotherapy while as 27 cases by vein chemotherapy and 36 cases by operation served as the control group. The expression of proliferating cell nuclear antigen (PCNA) were detected by Immunohistochemic S-P method,and cell apoptosis was measured by TUNEL.Result In gastric and colorectal carcinoma , compared with the venous chemotherapy group and the preoperative intraarterial infusion chemotherapy group ,the expression of PCNA in operation served group was significantly higher(p<0.05), compared with the the preoperative intraarterial infusion chemotherapy chemotherapy group, the expression of PCNA in venous group was significantly highe(rp<0.05). Compared with the venous chemotherapy group and the preoperative intraarterial infusion chemotherapy group ,the AI in operation served group was significantly lower(p<0.05), compared with the the preoperative intraarterial infusion chemotherapy chemotherapy group, the AI in venous group was significantly lower(p<0.05). From operation group to preoperative intraarterial infusion chemotherapy group,the expression level of Smad4 was positively correlated with AI(r= 1 p<0.01),and the negative correlation between the expression level of Smad4 and PCNA was observed (r=-1 p <0.01).Smad2 expression was not associated with AI(r>1 p>0.01)and PI (r<–1 p>0.01).Conclusion Preoperative intraarterial infusion chemotherapy could induce cell apoptosis and inhibit the cell proliferation in gastrointestinal carcinoma, it could be an effective accessorial chemotherapy method similar with vein chemotherapy.The PCNA and AI correlation with Smad4 that indicated Smad4 could induce cell apoptosis and inhibit the cell proliferation in gastrointestinal carcinoma in some mechanisms and the effect had been more significantly after preoperative intraarterial infusion chemotherapy.
