Study On The Changes Of Whole-blood Zn Cu Fe Se Concentration In Bone Marrow Transplantation Patients With Leukemia And The Relationship Between Their Changes And The Blood Sugar, Serum Enzymes And Oral Complications

Objective The trace element is closely related with the health of human body. The study aims are as follows:first,to study on the changes of whole-blood trace element concentration in bone marrow transplantation (BMT) patients with leukemia;second,to find the relationships between their changes and the blood sugar,serum enzymes and oral mucous membrane disease. Thus they provide a useful guidance of clinically using trace elements for preventing and treating the complications in BMT recipients and lacking of trace elements.Methods The concentrations of trace element zinc,copper,iron and selenium in whole-blood were determined by atomic absorption spectroscopy method in 20 patients with leukemia undergoing bone marrow transplantation and 20 normal control in June 2000 to January 2002.They did not show a significant in age,sex and region between the patients and the normal control. The blood samples of the patients were taken on days -5,0,+6 and +15 during undergoing bone marrow transplantation. At the same time,the levels of blood sugar,alkaline phosphatase,a-hydroxybutyrate dehydrogenase and lactate dehydrogenase in serum were determined by complete automatization biochemistry analysis method in 20 patients with leukemia undergoing BMT. Sequential changes of oral mucous membrane from days -5 to +15 were observed 20 patients with leukemia undergoing BMT,and simultaneously the registration forms of the complications were maked out.Results It showed that there were no significant difference (p>0.05) in the whole-blood concentration of trace element zinc,copper and iron between the transplantation group before pre-treatment and the normal control group,but selenium concentration in whole-blood samples of the transplantation groups before pre-treatment showed significant differences (p<0.01) as compared with that of normal individuals. There was significant difference (p<0.05) in the concentration of trace elements zinc,iron,selenium and-4-copper/zinc ratio between the transplantation group before pre-treatment and after pre-treatment,and no significant changes occurred in copper levels,but the whole-blood concentration of trace element iron and zinc gradually decreased from the pre-treatment to days +6 and then started to return after day +15,and simultaneous concentration of selenium,copper and copper/zinc ratio increased during the preconditioning and remained rising at days +6 and then started to return after days +15,but no significant changes occurred in copper levels. There was a significant relation between serum alkaline phosphatase activity and concentration of whole-blood trace element zinc and copper,and serum a -HBDH and LDH activity showed a significant relation with concentration of whole-blood trace element zinc,copper,iron and selenium. There was a significant relation between concentrations of whole-blood trace element zinc,copper,iron and selenium and blood sugar levels. The result indicated that copper/zinc ratio obviously changed before and after the pre-treatment of patients with leukemia undergoing BMT.Conclusion It indicated that there was metabolism perturbation of trace element in the patients with leukemia undergoing BMT. It was found that zinc and iron level decreased in the patients with leukemia after pre-treatment,and whole-blood selenium and copper/zinc ratio increased. The trace element is closely related with the blood sugar and serum enzymes. Oral mucous membrane disease is a common complication in BMT recipients. The occurrence,development,prevention of oral mucous membrane disease were related with the content of trace element zinc,iron and selenium. These changes may be due to undergoing pre-treatment high-dose chemotherapy or chemotherapy/radiotherapy to patients with BMT. We measured concentrations of whole-blood trace element zinc,copper,iron and selenium,blood sugar levels and serum enzymes activity during BMT for patients with leukemia in order to prevent and treat adverse effects of pre-treatment.
