Objective: Explore the function of melatonin in the pathogenesis and the prognosis of hypoxic-ischemic encephalopathy (HIE) and provide the pathophysiology basis for theraping HIE with melatonin.Methods: Assay the level of melatonin in the umbilicus bloods often normal term infants and in the femoral vein bloods of ten preterm infants and twenty modest HIE, twenty middle-severity HIE, ten asphyxia and ten aspiration pneumonia neonates in their acute phase and recovery phase respectively with radioimmunoassay. Then compare the difference of the melatonin level among these neonates.Results: 1, The melatonin level in modest HIE is higher than the normal and in middle-severity HIE has no difference to the normal, which is relation to the phase of HIE. In the acute phase the melatonin level is higher than middle-severity HIE and in the recovery phase there is no difference between them;2, In HIE the melatonin level of modest asphyxia is higher than the normal and severity asphyxia, which has interaction to the phase of HIE. In the acute phase it is obviously and in the recovery phase there isn't this action;3, In HIE merging with aspiration pneumonia the melatonin level is higher than it in without aspiration pneumonia and the normol neonates, which is relation to the phase of HIE. In the acute phase it appears and abolishes in the recovery phase;Conclussion: 1, In the acute phase the melatonin level of modest HIE, the HIE neonates with modest asphyxia and the HIE neonates with aspiration pneumonia is higher than the normal, which have no difference to the normal in the recovery phase.2, The melatonin level of middle-severity HIE neonates, the HIE neonates with severity asphyxia and the HIE without aspiration pneumonia have the lower tendency to the normal in the acute phase, in the recovery phase which have heighthing tendency.3, Melatonin have protection action on HIE. The prognosis of HIE neonates with heighthing melatonin level is better than with lower melatonin level4, Melatonin have therapeutical effect on HIE, which is better in the acute phase than in the recovery phase and is better in the middle-severity HIE neonates, the HIE neonates with severity asphyxia and the HIE without aspiration pneumonia than in the modest HIE, the HIE neonates with modest asphyxia and the HIE neonates with aspiration pneumonia.
