Purpose: This study investigated the method of operation and prognostic factors in the prognosis following resection of subsequent pulmonary metastases from osteosarcoma, with emphasis on the effect of neoadjuvant chemotherapy. Clinical material and method: From 1998 to 2002, we reviewed 10 patients treated with neoadjuvant chemotherapy and surgery. Results: The rate of follow-up was 100%. The 2-year survival rate after first resection of pulmonary metastases was 69.64% and the .3-year survival rate was 34.82%. Conclusion: We emphasize the importance of a thorough evaluation for metastases, a strict indication in selecting patients and a careful exploration. Relative risk factors for metastases were as follows: poor response to preoperative chemotherapy and/or lack of postoperative chemotherapy; tumor-containing resection margin (most likely to metastasize);latter detection and diagnosis.
