ObjectiveLaryngeal carcinoma is a common malignant tumor in otorhinolar-yngology, it was reported that the incidence rates increase year by year, and endanger people's health and living quality. The invasion and metastasis of tumor are the important causes which result in fail treatment and death of patient, and the mechanisms of invasion and metastasis of tumor have not been unknown yet. KAIlis a metastasis suppressor gene located on human chromosome 11p11. 2. It belongs to a structurally distinct family of cell surface glycoproteins. Decreased KAI1 expression has been observed in several common tumors, including prostatic, pancreatic, melanoma and colorectal cancers. There was still no report on the relationship between the KAI1 and tumorgen-esis and/or development of laryngeal carcinoma. We made the experiment to observe the expression of the KAI1 in Primary laryngeal squamous cell carcinoma ( PLSCC ) , laryngeal precancerous lesion (LPL) , vocal cord polyp (VCP) and normal laryngeal tissues (NLT) to investigate the relationship between the expression of the metastasis suppressor gene KAI1 and tumorgenesis, development of laryngeal carcinoma, and to look for the molecular marker for early diagnosis and predicting of aggressive and metastasis potential and assessing the clinical stage of PLSCC.Materials and MethodsWe studied laryngeal carcinoma specimens that were obtained from 84 patients who had undergone surgery at the First Affiliated Hospital of China Medical University between January 2002 and October 2002. The patients included 70 men and 14 women, ages 37 ~ 80 years (mean age,62. 8years) . According to the TNM classification of the International Union Against Cancer (1997), there were 19 patients with stage I , 35 patients with stage II , 16 patients with stage HI and 14 patients with stageIV. There were 34 supraglotic cancers, 42 glottic ones, 3 infraglottic ones and 5 tranglottic ones; well - , moderate - and poor - differentiated were 34, 32 and 18 respectively. In addition, 27 samples of of laryngeal precancerous lesion (LPL) andlO samples of vocal cords polyp (VCP) were also obtained for study and 10 samples of normal laryngeal tissues (NLT) were used as the normal control. Freshly removed tissue samples were fixed in 4% paraformaldehyde solution/0. 1M PBS for 1 ~2 hours and embedded in paraffin for standard histopathological evaluation and analysis of KAI1 mRNA expression by in situ hybridization. SJJL tissue sections were deparaffinized and rehydrated (in situ hybridization was performed as standard procedure). Negative control was performed without KAI1 probes. Staining intensity was estimated to be positive when it appeared clearly brown granule and analyzed quantitatively with system of colour multimedia image Metamorph Imager System V4.6. Statistical analysis was performed with t - test. The difference was considered significant when the P value was <0.05.Results1. There were brown granule in cytoplasm of NLT, VCP, LPL and PL$CC less or more.2.10 samples of NLT and 10 samples of VCP showed strong expression of KAI1 , the Integrated A Total of KIA1 in these two types of tissues were 136206. 8 ?6675. 536 and 135344.53 ?4113.418, respectively, with no statistically significant difference between them, and high prevalence of decreased expression of KAI1 in LPL and PLSCC compared with that in VCP and in NLT (P <0.01).3. There was a significant relationship between down - regulation of KAI1 expression with pathological grade, lymph node metastasesand and clinical stage (P <0. 01). and did not correlate with gender.Conclusion1. NLT showed strong expression of KAI1, larynx may be one of special organ which express KAI1 and there may be a correlation be-teen decreased expression of KAI1 and tumorgenesis and development of PLSCC.2. There were high prevalence of decreased expression of KAI1 in LPL and PLSCC, The down - regulation of KAI1 may be an early and common event in the progression of PLSCC.3.
