Objective: The purpose of study was to evaluate the feasibility of percutaneous sulfuric acid injection therapy(PSI) for the hepatoma and therapeutic effects on the implanted hepatic VX2 tumor in rabbit, to observed the biochemical changes in serum ,the radiological appearances and histopathological findings after PSI ,which provided an experimental proof for the treatment of hepatocellular carcinoma.Materials and Methods: 1 The induced necrotic area in vitro pig liver was measured after injection of absolute ethanol and 10%,20% , 30% dilute sulfuric acid in volume of 0.5ml,1ml,2ml, their relations between the lesion size and different concentration dilute sulfuric acid injected in different dose were observed, and comparison with the induced lesion size injection of ethanol in equal dose, to explore the appropriate concentration of sulfuric acid injected into the liver. 2.10 big white rabbits were randomly divided into two groups, 10% sulfuric acid was injected into liver at a dose of 1ml. The first group: the biochemicalchanges in serum were observed before and at immediate, 3 day, 7 day,and 15 day after the procedure. The second group: the radiological appearances, histopathological findings and their correlation were observed 3days, 7days, 15days and 30days after PSI. 3. 12 big white rabbits were randomly divided into two groups:ethanol group and sulfuric acid group。absolute ethanol and 10% sulfuric acid were injected with 0.5 ml respectively。All animals were sacrificed four hours after the procedure to measure the maximum area of lesion in two groups. 4. The implanted hepatic VX2 tumor in 16 rabbits were randomly divided into two groups :ethanol group and sulfuric acid group。Absolute ethanol and 10% sulfuric acid were respectively injected into VX2 tumor in volume of 0.5ml. The rates of tumor necrosis in two groups were compared statistically, and the MRI appearances of the tumor were observed before and after the procedure.Results: 1.The lesion size increased with the increasing concentration and injection dosage of sulfuric acid。The induced lesion area of 10% sulfuric acid injected was not significantly differently with that of 20% or 30% sulfuric acid injected in small amount(<2ml),but larger than that of ethanol injected in equal dose, which demonstrated that 10% sulfuric acid was more appropriately injected into liver and had greater necrotizing power than ethanol。2. The ALT and AST level in serum had transient elevation at immediate,but recurred to the same levelbefore the procedure, There was no significant elevation of ALB,ALP, γ-GGT,TBIL,DBIL and BUN level after PSI (P>0.05).Pathological changes indicated hepatocellular degeneration, coagulative necrosis , inflammatory cells infiltration and granulation tissue proliferation were developed。The necrotic lesion showed hypoattenuate without enhancement and clear margin in CT scanning,peripheral enhancement of the hepatic parenchyma surrounding the lesion was seen in arterial phase and disappeared gradually as time processed 。However,the lesion still kept hypoattenuate in portal phase on contrast-enhanced CT scanning。3. The lesion size in sulfuric acid group was larger than that in ethanol group,and there was significantly different between two groups (P<0.05)。4.The rates of tumor necrosis in ethanol group was significantly less than that in sulfuric acid group (P<0.05)。Electron microscopic observation of the treated region showed the tumor cells degeneration ,necrosis and apoptosis .The tumors were visualized on MRI scanning with a low-intensity,high-intensity,and high-intensity on T1-weighted ,T2-weighted and T2 diffuse-weighted image respectively before the procedure,but the treated region showed inhomogeneous signal 24hours after the procedure ,the tumor necrosis showed high-intensity, slight low-intensity on T1-weighted,T2-weighted image , without obvious enhancement。Conclusions:Dilute sulfuric acid is a new type of tumor cellsinactivation material.
