Objective To observe the changes of expression of basic fibroblast growth factor and transforming growth factor in combined transplantation of muscular-periosteal flap, bone marrow and spongy bone granule, discuss the function of bFGF and TGF-β in this process, and further expound the mechanism of bFGF and TGF-β repair the long bone defect. Method Thirty rabbits were adopted in this study, weight 2-2.5kg, randomly divided into 6 groups, 5 rabbits were in every group. 20mm bone defect model was made in each side of radius, randomly chose one side radius as experimental group, the other side as control group. muscular-periosteal flap, bone marrow and spongy bone granule was transplanted in experimental group, muscular-periosteal flap wastransplanted in control group. After 3 day, 1,2,4,6,8 week of operation ,the samples were taken. Histological examination and immunohistochemistry examination of bFGF and TGF-β was observed.Results Histological examination: In experimental group, the mesenchymal cells were gathered and could see a number of chondrocyte at 2nd week, and at 4th week, could see endochondrial ossification. At 6th week, could see a numbe of osteoblast and osteocyte, appeared marrow cavity. At 8th week, muscle tissue, through endochondrial ossification, changed into complete bone tissue, formed relatively complete long bones. In control group, at 4th week, could see a small number of chondrocyte in the muscle tissue, at 6th week, could see a small number of new formed bone at the cut ends of the cortex, muscle tissue changed into fiber tissue at 8th week, the number of new formed bone increased a little, but bone defect was not repaired completely there was much of fiber tissue in the part of bone defect. Immunohistochemistry examination: in experimental group, at 2nd , 4th , 6th ,chondrocyte, osteoblast, and osteocyte highly expressed bFGF and TGF-β, at 8th week osteocyte still expressed, but the number of expressing cell decreased. Control group, at 4th week,a small number of chondrocyte expressed bFGF and TGF-β, at 6th and 8th week osteocyte in the new formed bone of cut ends of cortex still expressed, but compared with experimental group, the number of expressing cell, had a significant difference(p<0.01). Conclusion 1.bFGF and TGF-β can encourage the inducing osteogenesis, combined transplantation of muscular- periosteal flap, bone marrow and spongy bone granule highly express and release bFGF and TGF-β in the part of bone defect in the early stage, so can repair the bone defect earlier and better. 2. Because of the interaction of bFGF, TGF-β, BMP, PDGF and so on, combined transplantation of muscular-periosteal flap, bone marrow and spongy bone granule can repair bone defect earlier and better.
