Background:Primary hepatocarcinoma is a common malignant tumor of digestive tract, with high-incidence-rate, low-resection-tate and high-recurrence-rate. During more than the recent 10 years, as a microtherapy for carcinoma inactivation in situ,ultrasound-guided interventional therapy for hepatocar- cinoma has gained great progress. According to the therapeutic types it was divided into 2 kinds: 1. local injection therapy, 2. local interstial therapy. Both display good anti-tumor activities, meanwhile, every therapy has its advantages and disadvantages. This study wants to achieve the purpose of targeting administration, slow release by changing the form of the dose and the way of the administration. we inject the self-made carboplatin-loaded PLGA microspheres into VX2 liver tumor in rabbits by ultrasound-guided, observe the anticancer activities, access the effectiveness and feasibility.Objective:1. To make animal models of implanted VX2 liver tumor in rabbits, which are potenial used for intervenional therapy. To observe the tumor's biological features with ultrasound and pathology.2. To make carboplatin-loaded PLGA microspheres for intratumoral injection by uniform design method.3. To evaluate the anticancer activities of ultrasound-guided microspheres intratumoral injection for VX2 liver tumor in rabbits by ultrsound and pathology.Methods:Animals which were inoculated of VX2 liver tumor were divided into 3 groups randomly, and begun to be therapied at 2 weeks after inoculation.ultrasound-guided microspheres,free-carboplatin,saline intratumoral injection into VX2 liver tumor respectively. Once every 3 days. To evaluate the growth of tumor with ultrasound: measure the diameter, observe the tumor's sonograph and flow images. 5 rabbits of every groups were put to death after 1 week, tumor samples were pathological examined. To analyze tumor necrosis degree of every group, at the same time, observe hepatic function,hemogram. To observe the law of the changes during pre- and post-treatment. Data were analyzed by statistics.Results: 1. The study showed tumor grew productivly during 2~4 weeks after inoculation and tumor's body increased rapidly. 4 weeks later degeneration and necrosis at the center of tumor can be seen, peripheral lymph node metastasis were aslo be seen. Lung metastasis were occured later. Survival time of rabbits bearing carcinoma was 41.5±3.2days.2. Self-made microspheres loaded 10.16% carboplatin with diameter of 10μm~100μm. 44.4% carboplatin had been released during 24 hours. Release becamed slowly 60 hours later, maitaned more than 10 days.3. During ultrasound-guided microspheres intratumoral injection, the B mode images of tumor were enhanced, blood perfusion was greatly reduced. Tumor grew slowly, the volum was less than the controled groups evidently.4. When the rabbits dead, autopsy showed abdomen metastasis in all rabbits in saline group, lung metastasis occurred in 3 rabbits. In free-carboplatin group 2 rabbits had abdomen metastasis and no lung metastasis. In microspheres group tumor were limited to liver and no metastasis. 5. Pathological examine showed tumor necrosis severely in microspheres group. The structure of neoplasia cell destruct completely at the center of tumor.6. During pre- and posttreatment hepatic function and hemogram had no evident changes in every group.Conclusion:1.Animal models bearing VX2 liver tumor setting up by investment have steady biological feature, with a high successful rate, which suit to interventional therapy study and effect evaluation.2. The physical feature of self-made microspheres suits to injection.3. Microspheres intratumoral injection guided by ultrasound can evidently inhibit the tumor growth and tumor metastsis. The therapentic efficacy is better than free-carboplatin.4. Intratumoral injection can reduce carboplatin entering blood circulation, thus the poison and ill effect of the body is decreased.5.
