Objective:Combined requirements of posterior labyrinth and partial labyrinthectomy surgery,measure the correlative data at specimen, study the anatomy of temporal bone andCerebellopontine angle area, to provide anatomic data of the surgery, and try to afforda suitable scheme to carry out the correlative surgery as well as protect the function.Methods:To divide the anatomic area into several parts: sigmoid sinus , jugular bulb,semicircular canal, internal acoustic meats, cerebellopontine angle area, duramater.Simulated the partial labyrinthectomy and endoscopic surgery on 20 adult cadavericspecimens, measured correlative data, observed correlative anatomic structure. Alldata analysied by SAS.Results:1. 10%(4 of 40) distance between sigmoid sinus and surface of temporal bone is 7 cm. 7.5%(3 of 40) width of sigmoid sinus is 15mm. 12.5% (5 of 40) distance between sigmoid sinus and posterior wall of external acoustic meats is 10mm.2. There is a remarkable difference between the height of two sides of jugular bulb. Right is higher than left. 22.5% are higher than the inferior edge of posterior semicircular canal. 5 % are higher than the superior edge of endolymphatic sac.3. By retrolabyrinthine approach, rectangle incision of duramater got 116.80 mm2 surgery scope. "T" incision of duramater got 124.68mm2 surgery scope. The excellence of latter is that its incision can be adjusted optional.4. 17.5% of specimens have angled variation of the back wall of petrous bone. An anterior sigmoid sinus tend to be observed when the distance between petrous back wall and external auditory canal is shorter.5. 11/40 (27.5% ) posterior lip of internal acoustic meatus cover facial nerve more than 1 /3. 9/40 (22.5%) anterior inferior cerebellar arterial bow lie in lateral of facial nerve. 3/40(7.5%) have both of them.6. Posterior semicircular canal resection could get 15 10mni surgery scope. It couldtreat cerebellopontine angle lesions and save audition and balance, but could not treatinternal acoustic meats lesions. Super-posterior semicircular canal resection s scopeis wider than posterior semicircular canal resection and could treat internalacoustic meats lesions too.Conclusion:We get the scope of the two different incisions of duramater. Study the effect of thesigmoid sinus, jugular bulb and Cerebellopontine angle area to retrolabyrinthineapproach surgery. Measure partial labyrinthectomy correlative data. Discuss how toprotect important structure. The research provide detail information about the twosurgery.
