[Background and Objective]Mycoplasma is the organism that resides ubiquitously at the cell membrane or internalized into the cell. Mycoplasma infection may result in some diseases providing other factors. Considerable advances have been made towards a better understanding of mycoplasma's biological behavior and pathogenesis, but the oncogenic potential of mycoplasmas was only recently realized, saying nothing of the relationship with laryngeal carcinoma. The interaction of mycoplasma with its lodger may startup the signal transmition from cell membrane to nuclear, which may change the expression of many genes, induce many cell factors and effect on cell hyperplasia and differentation. The continuous chronic infection of mycoplasma may arouse multiphase malignant transformation of cell and give birth to neoplasma. Some mycoplasma protein facilitate neoplasma metastasis, on the other hand, many kinds of enzyme of mycoplasma can restrain cell growth of tumor intensively. The incidence of laryngeal carcinoma incline to go higer recently, but its pathogenesis is unknown. The relationship between laryngeal carcinoma and many kinds of biological factors such as smoking, drinking and human papillomavirus has been noticed in our previous research. The present study was to determine the prevalence of mycoplasma in different kinds of laryngeal tissues and find the relationship between mycoplasma infection and the pathogenesis, development and prognosis of laryngeal cacinoma. In the mean time, the expression of proliferating cell nuclear antigen and CD44 protein were detectived so as to find the interaction of many biological factors bring to the laryngeal carcinoma and provide a new molecular biological foundation on which we can investigate the biological factors which related to the pathogenesis, development and prognosis of laryngeal cacinoma.[Methods]The 256 cases of different archived paraffinembedded laryngeal tissues samples were obtained by surgery or biopsy in ChangZheng hospital from 1991 to 2001.All samples were divided into six groups, the pathogenic and control groups. In pathogenic groups there were 105 cases of laryngeal carcinoma and 31 cases of neck metastatic lymphnode named as group A and group B respectively. In control groups, there were 18 cases of precarcinoma lesions(including 9 cases of chronic hyperrophic laryngitis, 4 cases of adult laryngeal papilloma and 5 cases of laryngeal leukoplakia), 53 cases of vocal cord polypus, 20 cases of laryngeal tissue taken from the area 0.5-1.0 cm adjacent to the carcinoma and 29 cases of normal tissues taken from the area more than 1.5 cm away opposite to the carcimoma of larynx, which were named as group C, group D, group E and group F in turn. Tissue samples were all verified pathologically. Monoclonal antibody PD4, which specifically recognizes a distinct protein from mycoplasma hyorhinis, was used to detect mycoplasma infection in these tissues with Evision immunohistochemistry. The expression of proliferating cell nuclear antigen(PCNA) and CD44 protein in different laryngeal tissues were detected also in this work.[Results]The results turned out to be as follows:1.Mycoplasma was presented in 44.76%(47/105)of laryngeal carcinoma tissue, which is more frequently founded than in neck metastatic lymphnode 41.94%(13/31), precarcinoma lesions 16.67%(3/18), vocal cord polypus 13.20%(7/53),normal tissue adjacent to the carcinoma 5.00%(1/20) and normal tissue opposite to the carcinoma 3.45%(l/29).2. The positive rate of mycoplasm infection are 19.23%(5/26), 46.67%(28/60) and 73.68%(14/19) from the first histological grade to the third grade in laryngeal carcinoma.3. The positive rate of mycoplasm infection in third and fourth stages laryngeal carcinoma is 58.21%(39/67), the first and second stages is 21.05%(8/38).4. Mycoplasma was presented in 64.52%(20/31) of laryngeal carcinoma with cervical metastasis, but without cervical metastasis there is only 36.49%(27/74).5.
