[Background and Objective] Gastric carcinoma is one of the most common malignant diseases in the world. Recently, with progressing in study of gastric carcinoma, outcome of treatment in gastric carcinoma has improved. Due to many factors relate to prognosis of gastric carcinoma, the 5-year survival rate in patients after curative surgical resection hovers around 50%. Observation and analysis of prognosis parameter are helpful to determine and implement of optimal treatment plan that maybe improve survival rate. This paper aims to study clinical significance COX-2 protein in stomach cancer and analysis the relationship among expression of COX-2 protein and site, differentiation, pathologic stage, lymph nodemetastasis and its prognostic significance in gastric carcinoma. [Methods] Expression of COX-2 protein was detected byImmunohistoc-hemical stain in tissue of cancer which was final diagnosed from 67 postoperative gastric carcinoma sample in our hospital from August 1997 to June 2002. According to follow-up, statistical analysis of the correlation between COX-2 expression in the tumors and pathologicaldata were calculated with x test, Spearman correlation test respectively, Kaplan-Meier Survival curves were generated, and the log-rank statistic was used to determine the differences between the curves, Cox regression multivariate analysis was performed on the entire patient cohort using age, differentiation, T stage, Lymph node, and COX-2 was considered an independent variable. The analysis was done by SPSS version 10.0 software.[Results] Our study showed COX-2 protein in 67 gastric carcinoma sample, fifty-two (77.6%) were positive and 15( 22.4%) were negative. There was significant correlation between COX-2 protein expression and site, pathologic stage, lymph node metastasis (P<0.05). According to site of gastric carcinoma, the positive of COX-2 protein in cardia of stomach was 50%, in body of stomach was 71.4%, in gastric antrum was 88.9%, according to T stage , COX-2 protein positive T1(52.6%), T2(78.6%),T3(88%), T4(100%), according to N stage , COX-2 protein positive N0(65.2%), N,(68.4%), N2(94.4%), N3(100%). There was no correlation between COX-2 protein expression and grade differentiation (p>0.05). The survival analysis showed the 3-years survival rate in the patients with COX-2 expression was much lower than the rate in these patients without COX-2 expression, and the Cox model of multivariate of epidemiology showed COX-2 expression play role in gastric carcinoma prognosis. [Conclusion] From our study, we can draw a conclusion that it is significant correlation between abnormal expression of COX-2 protein and tumor site, pathologic stage, metastasis in gastric carcinoma, and expression of COX-2 plays an important role in prognosis of gastric carcinoma.
