Since nineteenth century Pasteur found that garlic had some antibiotic activity, many researches have been focused on garlic. Modern science approved that garlic functioned as antibiotic, anticancer and immune system activator.Helicobacter pylori ( H. pylori) infection associated with sorts of gastric diseases. WHO advised H. pylori as class I cancer generator, especially for gastric cancer. To eradicating H. pylori, the most common regimen is triple therapy, including proton pump inhibitor ( PPI) or a bismuth compound plus two of the three antibiotics: clarithromycin, amoxicillin and metronidazole. Its efficacy is determined by three factors; compliance, treatment duration and especially the prevalence of metronidazole resistance in the population. Allitridi retorted from garlic maybe a hopeful new agent in treating H. pylori infection.Available anti-H. pylori component in garlic include Allicin, Allitridi and Allistatin. Because of different purification methods, different preparation and e-ven different name, garlic had a dilemmatic conclusion in treating H. pylori.H. pylori pathogenetic gene, including cagA, vacuolating cytotoxin A ( va-cA) and et al, had been variated. Different subgroup of H. pylori localized in different part of the world. There was little report on susceptibility of subgroups of H. pylori strains to allitridi. Our study tested susceptibility of H. pylori stains to allitridi and metronidazole with electrophoresis on agarose gel, compared the susceptibility of H. pylori stains to allitridi to that of the metronidazole, observed the combined effect of the two on H. pylori, discussed the susceptibilities of different subgroups of H. pylori strains to allitridi and metronidazole, in order to provide useful reference on clinical approach to eradicate H. pylori.METHODSMICS of allitridi susceptibility on 51 H. pylori strains were determined by agar dilution methods. MIC50 was also calculated. H. pylori cagA and vacA (s1, m1b, m2) genotypes were classified by PCR method.RESULTSIn 51 H. pylori strains ,cagA + and cagA- types occupied 90.20% and 9. 80% ; vacAsl + and vacAsl- 62. 75% and 37. 25% ; vacAmlb + and va-cAm1b- types occupied 64. 71% and 35. 29% ;vacAm2 + and vacAm2- types occupied 74. 51% and 25. 49% . H. pylori strains in high risk area of gastric cancer were more susceptible to allitridi than those in low risk area of gastric cancer . Multiple H. pylori strains were more susceptible to allitridi than mono-strains. H. pylori strains with vac Am1b + genotype were more susceptible to allitridi than those with vacAmlb- genotype. H. pylori strains with vacAm2 + genotype were more susceptible to allitridi than those with vacAm2- genotype. H. pylori strains with cagA +/vacAsl + mlb + genotype were more susceptible to allitridi than those with cagA +/vacAs1-m1b- genotype.CONCLUSIONH. pylori strains with cagA +/vacAs1 +m1b + genotype were more susceptible to allitridi than those with cagA + vacAs1-m1b- genotype. H. pylori strains with cagA + /vacAs1 + m1b + genotype are suggested to be eradicated preventively , in order to avoid severe gastric diseases.
