Obstructive sleep apnea syndrome(OSAS) is a kind of complicated disease with high incidence that has a potential lethal danger.It results from respiratory discontinuous decrease or pause due to the upper airway narrow or emphraxis during sleep. The treatments for OSAS includes surgical therapy and non-surgical therapy. Mandibular advancement device is a kind of non-surgical treatment .This research aims to study effects of three body position in the way of facial formation and structure relation of soft and hard issue around airway for non-snore'ers and OSAS patients with mandibular advancement device.1. The cephalometric analysis before and after wearing mandibular advancement device for non-snore'ers at different body posturesObjective: To study the effects of mandibular advancement device on upper airway at three different body postures. Methods:Ten non-snore'ers were diagnosed by overnight polysomnographic and aphalomertic analysis with and without mandibular advancement device at different postures. Results: Ten males were all normal by overnight polysomnographic.After wearing mandibular advancement device at the same body posture,MP-SN angle,H-Me distance and oropharynx distance increased, SPT decreased; Before and after wearing mandibular advancement device,H-Mp and H-Me distance decreased at supine and relaxing states than at sitting; SNB angle and U-Pu distance increased at relaxing than sitting. Conclusion: The dimension and shape of pharyngeal cavity and soft tissues were changed by mandibular advancement device at three different body postures.2. Pilot study of treating OSAS with mandibular advancement deviceObjective: To investigate the feasibility of mandibular advancement device for the treatment of obstructive sleep apnea syndrome(OSAS) and the effects of three body postures on the upper airway. Methods: Ten OSAS patients were diagnosed by overnight polysomnographic and aphalometric analysis with mandibularadvancement device in different postures. Results: Following the treatment with mandibular advancement device,the position of mandibular was changed to protrusive one,with corresponding changes in the shape of pharyngeal cavity and the positional relation of it with surrounding soft tissues,among all the 10 cases.AHI was decreased from 10.74 4.32times/h to 3.65 3.45times/h. Conclusions: Mandibular advancement device is an effective way to treat slight OS AS. The dimension of airway was changed by mandibular advancement device at different postures.3. Contrast research of cephalometric measurement at different body postures for non-snore'ers and OSAS patientsObjective: To investigate the influence on upper airway at different body posture between non-snore'ers and OSAS patients by the use of aphalometry. Methods: Ten non-snore'ers and ten OSAS patients were diagnosed by aphalometric analysis at different body posture. Results: Compared with non-snore'ers at the same body posture, OSAS patients' ANB angle, the thickness of soft palate and hyoid vertical distance increased, oropharynx distance decreased; SNB angle and MP-SN angle of non-snore'ers decreased at supineand relaxing than sitting, the thickness of soft palate increased; H-C3 distance and PAS distance decreased. H-C3 distance of OSAS patients also decreased. Conclusion: Non-snore'ers have a significant difference with OSAS patients in the way of facial formation and structure relation of soft and hard issue around airway at different body postures.
