Clinical Study On The Treatment Of OSAHS With Mandibular Advancement And Snore Stopping Device And Its Portable Development Of Mandibular Protrusive Localizer

Objective:The mandibular oral appliance (MASSD) in treatment of mild and moderate OSAHS patients,preoperative and postoperative polysomnography (PSG),Berlin questionnaire (BQ),Epworth Sleepiness Scale (ESS),comfort questionnaire,comparative study, to evaluate the clinical efficacy of treatment of OSAHS MASSD.The other through the study of the mandibular oral appliance on OSAHS mandibular repositioning, combined with the observation of clinical cases, aiming at the shortcomings of traditional mandibular repositioning in the clinical application and development of portable mandibular positioner.Methods: 1. The study included 30 patients with mild to moderate OSAHS. MASSD was used to treat patients with OSAHS, and preoperative and postoperative clinical comparative study was performed. Objective To observe the curative effect of PSG index as the monitoring index: apnea hypopnea index (AHI), snoring frequency, the lowest oxygen saturation (LSa02); subjective efficacy index: Berlin questionnaire(BQ),Epworth Sleepiness Scale (ESS),comfort questionnaire. The observation time was six months after wearing MASSD,and the data of each observation index were statistically analyzed, with P < 0.05, the difference was statistically significant.2.Discuss the deficiency of several common mandibular protrusive positioning method, combined with Kunming University of Science and Technology and Shanghai IBM company, the stepper motor, stepper motor drive, power supply,Bluetooth module, Android intelligent mobile phone system, pulse oximeter device refit and programming, complete portable mandibular positioner, according to the patient's subjective feeling and the monitoring data, computer aided determination of mandibular distance, the realization of "personalized" mandibular distance adjustment,improve the treatment effect and comfort degree.Results:1. Thirty cases of mild and moderate OSAHS patients treated with mandibular snoring, were observed after 6 months follow-up, the objective results showed that:after treatment, the average AHI index dropped to (10.24 + 4.25) / h; the lowest oxygen saturation (Lsao2) significantly increased, greater than or equal to 91%. By statistical analysis, the curative effect was positive (P<0.05). Because the number of snoring did not obey the normal distribution,the results were expressed by P50 (P25,P75), and the rank sum test was 0.001 before and after treatment. The questionnaire survey results show that: in this experiment, 30 cases of patients treated with snoring after 27 cases out of 2 cases of patients with apnea risk,although not out of apnea risk,but eased; 30 patients more comfortable 12 cases, accounting for 40%, accounting for 43.33%,13 cases of comfort and discomfort in 3 cases,10%,obvious discomfort in 2 cases, accounting for 6.67%, patients have teeth pain, dry mouth, the morning after the occlusion of discomfort, increased saliva,cheek,joint swelling and other symptoms, 5 cases of discomfort patients with mandibular distance change, improve comfort, are willing to wear snore stopper.2. Self-made portable mandibular protrusion positioner.Conclusions:1. MASSD are effective methods for the treatment of mild to moderate obstructive sleep apnea hypopnea syndrome (OSAHS).2. The amount of mandibular protrusion is closely related to the curative effect of the oral snore stop device, but the small amount of protraction can not achieve the therapeutic effect.If the assembly causes discomfort or even side effects, it should be noted that MASSD should be used for mandibular protraction Further research is needed.3. Slef-made portable mandibular protrusive positioner to realize the regulation of individualized mandibular protrusion distance and improve the therapeutic effect Fruit and comfort. It is expected to be popularized in clinical practice.
