Objective : Subarachniod hemorrhage (SAH) was one of most common cerebral vascular diseases. Cerebral Vasospasm (CVS) was its important complication, which was an important problem of neurosurgery and an essential reason for death and disability. According to present report, CVS was found in 70% patients with SAH by angiography, which caused to brain ischemia, edema, infarction and softening. Finally, the patients would occur disturbance of consciousness, hemiplegia, aphasia, or death. CVS include acute cerebral vasospasm (CVS) and delayed cerebral vasospasm (CVS).The mechanism of delayed CVS was more complicated. Morphological change of smooth muscle cells after that could be observed by pathology. Once the change occurred it was difficult to take turn for CVS and reaction of the vessel to vasodilatation medicine was worse, which brought some difficulties for clinical treatment. For a long time, neurologist had been exploring the mechanism of delayed CVS after SAH and effective methods in prevention and treatment. Many neurologists attached importance to the role of free radical, endothelins and nitrogen monoxide in delayed CVS. Superoxide dismutase (SOD) was a kind of metalloid protein and cleaning substance of free radical with negative charge. Molondialdehyde (MDA) was the stable metabolite of lipid peroxidation products. The ratio of MDA/SOD reflected the ability of idation and anti- idation It was also an important index to reflect metabolism of free radical in body. The main component of Acanthopanax senticosus (AS) was senticoside. There was no report that the effective component of AS reduced delayed CVS after SAH and the concerned mechanism. This study was to explore the treatment and prevention effectand possible mechanism of AS to delayed CVS after SAH by detecting the contents of SOD,MDA and mean velocity (Vm) of blood flow of middle cerebral artery (MCA) at different time,Methods: The patients with SAH were divided into two groups accoroting to Hunt-Hess criterion. One was AS treatment group: routine therapy + AS injection ( 7mg/kg) per day, 14 days altogether; The other was SAH control grorp: giving routine therapy only. All patients were taken lumbar puncture to cllect cerebrospinal fluid(CSF) before treatment and at the 7th day, the 14th day after treatment respectively. CSF of the patients with lumbar anesthesia was taken as blank group. Activity of SOD was detected by xanthine oxidase technique .Content of MDA was detected by thio-barbituric acid technique. The mean velocity of blood flow of MCA was detected by transcranial Doppler at different time. Statistical analysis of the data was made by Spss 10.0. One way ANOVA was adopted to compare the difference of these groups. The difference between two groupsResult :1 , SOD activity of CSFBefore treatment, there was no significant difference (P=0.892) between SOD activity in AS group and that in SAH group. But SOD activity in two groups was markedly decreased, there was a significant difference (P<0.05 ) compared with that in blank group respectively.After 7 days, SOD activity in AS group and SAH group significantly decreased compared with blank group respectively (P< 0.001 ) . But SOD activity in SAH group decreased more significantly than that in AS group (P<0.001 ).After 14 days, SOD activity in AS group increased significanly compared with that in SAH group(P < 0.001 ) .Furthmore SOD activity in AS group was almost returned to normal, there was no significant difference between AS group and blank group (P< 0.001). 2, MDA Level Of CSFBefore treatment, there was no significant difference (P=0.992)between MDA level in AS group and that in SAH group.But MDA Level in two groups was markedly increased, there was a significant difference (P <0.05 ) compared with that in blank group respectively.After 7 days, MDA Level in AS group and SAH group significantly increased compared with that in blank group respectively (P< 0.001 ) .
