Objective: To find out the distinctive superiority of calcium sensitizer sulmazole in curing stunned myocardium by comparing its effects on ischemia/reperfusion stunning with that of β-adrenergic acceptor agonist dobutamine.Methods: 24healthy rats were divided into 3 groups randomly. After they were made into Langendorff isolated rat heart models, these three groups were perfused with K-H solution, K-H solution + sulmazole(1μmol/L) or K-H solution + dobutamine( 111 mol/L) respectively. LVESPs, LVDPs, dP/dts, HRs were measured before ischemia, before and at 5, 10, 20, 30, 60min after the beginning of medicine perfusion. Q1ac andQCK-MB were measured and calculated before and at 30 and 60min after the beginning of medicine perfusion. The electrocardiograph was monitored throughout the experiment to find out the incidence of serious arrhythmia. Myocardial ultrastructure of left ventricular subepicardium was examined by electronic microscope.Results. Both sulmazole and dobutamine were able to increase LVESP and dP/dt and decrease LVDP compared with control group. Sulmazole increased dP/dt more and its Q1ac was less compared with dobutamine group. The serious arrhythmia incidence of sulmazole group was less than that of dobutamine group. The mitochondrial impairment of dobutamine group was more serious than that of sulmazole or control group. There was no significantly difference in QCK-MB of these three groups.Conclusions. 1) Calcium sensitizer sulmazole is an effective medicine in curing stunned myocardium. 2) Compared with -adrenergic acceptor agonist dobutamine, sulmazole has distinctive superiority in improving the ventricular function andenergy metabolism of stunned myocardium, decreasing the incidence of serious arrhythmia and preventing myocardial cell from being damagde.
