Hypertention is a common disease caused by heredity and environment in the whole globe. Heart is the organ which is the most easily damaged by hypertention. A lot of papers suggest that left ventricular hypertrophy is an independent risk factor which determines the progress of hypertention. The histopathology change lies in left ventricular remodeling, which characters include the hypertrophy of cardiocytes, the fibrosis of interstitial cells, the change of left ventricular weight and shape. Among those change, myocardial fibrosis caused by the hyperplasia of extracellular matrix , especially collagen protein, has become one of the hot topics in recent cardiovascular fields. Some studies suggest that angiotensin converting enzyme inhibitor (ACEI) can decrease blood pressure effectively, prevent and reverse myocardial fibrosis, the mechanism of which is related to the influence on the rennin-angiotensin- aldosterone system (RAAS). Some clinical studies have confirmed Biejia Ruangan recipe can reverse hepatic fibrosis, but its influence on myocardial fibrosis has not been studied in the world.Objective: Through the comparative observations to the controls, to study the effects of Biejia Ruangan recipe on left ventricular weight, myocardial fibrosis and the changes of blood pressure, ventricular function in spontaneously hypertensive rat (SHR), we also make anelementary discussion about the mechanism of Biejia Ruangan recipe.Methods: 12-week-old SHRs were divided randomly into 5 groups: SHR-C(untreated), SHR-Y(treated with Enalapril), SHR-B1(treated with low dose of Biejia Ruangan recipe) SHR-B2(treated with moderate dose of Biejia Ruangan recipe). SHR-B3(treated with high dose of Biejia Ruangan recipe), each group consisting of 10 rats. And 10 normal SD rats were used as negative control. After 10-week treatments, we measured the left ventricular function with artery intubation, determined the HWI and LVMI; we also observed the change of structure and shape in the myocardial cells with HE and Masson, observed the change of ultrastructure in the myocardial cells with transmission electron microscope, determined collagen volume fraction (CVF) and perivascular circuferential area (PVCA) by pathological examination with computed processing; we determined collagen contents with spectrophotometry technique, examined the expression of collagen I and III with immunity histochemistry; we determined the levels of serum Procollagen type I carboxyterminal (P I CP) and peptide type III procollagen (PCIII), and the levels of plasma Angiotensin II (Ang II) and Aldosterone (ALD) with radioimmunoassay technique.Results: 1. Compared with SD rats, the SBP, LVEDP, HWI, LVMI of SHR were significantly increased, the + dp/dt max were significantly decreased. After the treatment of Enalapril, the SBP, LVEDP, HWI, LVMI of SHR were significantly decreased, the + dp/dt max were significantly increased. After the treatment of Biejia Ruangan recipe, the SBP, +dp/dt max, HWI, LVMI of all groups had no significant change; the -dp/dt max of moderate dose and high dosegroups were decreased; the LVEDP of all dose groups were increased.2. Compared with SD rats, the CVF, PVCA of SHR were significantly increased. After the treatment of Enalapril, the CVF, PVCA of SHR were significantly decreased. After the treatment of Biejia Ruangan recipe, the CVF, PVCA of all groups were decreased.3. Compared with SD rats, the myocardial collagen, collagen I and III, serum P I CP, PCIII of SHR were significantly increased. After the treatment of Enalapril, the myocardial collagen, collagen I and III, serum P I CP, PCIII of SHR were significantly decreased. After the treatment of Biejia Ruangan recipe, the myocardial collagen, collagen I and III, serum P I CP, PC III of all groups were decreased.4. Compared with SD rats, the plasma Ang II, ALD of SHR were significantly increased. After the treatment of Enalapril, the plasma Ang II, ALD of SHR were significantly decreased.
