Objective To investigate the changes of vascular endothelial growth factor (VEGF) and interleukin 6 (IL-6) in serum and follicular fluid (FF) of patients undergoing assisted reproductive technique (ART), and evaluate the relationship between VEGF, IL-6 and ovarian hyperstimulation syndrome (OHSS).Methods Ten moderate or severe OHSS patients and six mild OHSS patients and twenty nine non-OHSS cases were enrolled prospectively from ART program. The VEGF and IL-6 levels of FF on the day of oocytes pick up (DOPU) and of serum on the day of human chorionic gonadotrophin (hCG) injection (DhCGI) and DOPU and embryo transfer (DET) were measured by enzyme-linked immunoabsorbent assay, then analysis the relationship of the clinical data and detected indexes among the three groups.Results The VEGF and IL-6 levels of serum on the DhCGI, DOPU and on the DET were not significantly different among the three groups (P > 0.05 ) . The concentration of VEGF in FF was 1485.08 442.28 pg/ml in the moderate or severe OHSS group, whereas it was 1027.28 + 306.82 pg/ml in the mild OHSS group , and it was 1000.13 383.89 pg/ml in the non-OHSS group, the difference was statistically significant ( P <0.05). The concentration of IL-6 in FF was 48.66 28.69 pg/ml in the moderate orsevere OHSS group, whereas it was 12.03 + 6.47 pg,ml in the mild OHSS group , and it was 9.13 + 6.52 pg,ml in the non-OHSS group, the difference was statistically significant ( P < 0.05). The concentration of estradiol (E2) in serum on the day of hCG injection and the number of ovum retriveled much higher in the moderate or severe OHSS cases and mild OHSS cases than that in the non-OHSS cases (P < 0.01) .The concentration of basal follicle stimulating hormone (FSH), luteinizing hormone (LH) and basal ? were not significantly different among the three groups (P > 0.05) .Conclusion The concentration of VEGF and IL-6 in follicular fluid of moderate or severe OHSS cases is higher than that of controls, supporting the role of VEGF and IL-6 as a mediator of OHSS. Therefore VEGF and IL-6 in follicular fluid are forecast index of OHSS. According to the concentration of VEGF and IL-6 in follicular fluid, the E2 Levels of serum on the day of hCG injection and the number of ovum retriveled, we can forecast OHSS more accurately. The clinical pregnancy rate of OHSS cases is higher than that of non-OHSS cases.
