Objective: To investigate the therapeutic effects of domestically manufactured Ulinastatin on acute pancreatitis and explore its mechanism. Methods: A total of 199 patients with acute pancreatitis were randomly divided into the three groups and treated with Ulinastatin (n=63), Sandostatin (n=70) and Gabexate mesylate (n=66) respectively. In the group of Ulinastatin, patients had Ulinastatin 100000 IU bid ivd for 3d, then reduced to 100000 IU qd ivd for 7d. In the group of Sandostatin, patients had Sandostatin0.1 mg tid im for 10d.In the group of Gabexate mesylate, patients had Gabexate mesylate 100 mg tid ivd for 3d, then reduced to 100 mg qd ivd for 7d. After all the patients were treated for 5, 7 and 10 days, respectively, the changes in symptoms, physical signs and laboratory data were observed. χ2 test was used for statistical analysis. Results: In the group of Ulinastatin the cure rate was 69.8%, the effective rate was 88.8%; in the group of Sandostatin the cure rate was 71.4%, the effective rate was 90.0%; in the group of Gabexate mesylate the cure rate was 56.1%, the effective rate was 72.7%. There is no significant difference among the three groups as the cure rate concerned (P>0.05). But the effective rate of Ulinastatin and Sandostatin are significantly higher than Gabexate mesylate (P<0.05). In the group of Ulinastatin most patients'sign and symptom relieve among 3-5 days and after the course of treatment all patients'previous symptoms such as abdominal pain and distention disappeared, but in the group of Gabexate mesylate, after the course of treatment 4.5% patients had abdominal pain, 5.2% patients had abdominal distention, 4.8% had tenderness, 6.2% had rebound tenderness. The blood and urine amylase recovered to normal more rapidly in the group of Ulinastatin contrast to the group of Gabexate mesylate (P<0.05). The treatment of Ulinastatin can shorten the course of treatment and patients can take less time in hospital.The curative effect of Ulinastatin is similar to that of Sandostatin which is the most effective drug for acute pancreatitis at present. As Ulinastatin is abstracted from human urine, it has no immunogenicity, then it is safety to use Ulinastatin. There is no obvious toxic side effect in the course of treatment. The Ulinastatin is domestic, so it is convenient and less- cost for the patients to use it. Conclusion: Ulinastatin is an effective and safe agent for treatment of acute pancreatitis.
