Glaucoma is a common disease with the damage of optic nerve and visual field. In which the most common is primary acute angle-closure glaucoma. The examination of visual field is the most common used measure for many years. But many studies show that before the visual field defects the damage of optic nerve has already occurred. So we need more sensitive examinations to detect the damages of the early stage of glaucoma. We use Optec 3500 vision tester examine the changes in contrast sensitivity(CS) and color vision in patients with primary acute angle-closure glaucoma which provide the basis for early diagnosis and judgement of visual function damage degree.Objective: The study was designed to observe the changes in contrast sensitivity(CS) and color vision in patients with primary acute angle-closure glaucoma.Methods:We collect 52 patients (67 eyes) with primary acute angle-closure glaucoma who hospitalize in our hospital from 2003-3 to 2004-3 , including 23 men and 29 women; age 43-72, median 56.27±10.06 years . included in this study 35 (35 eyes) patients with early primary acute angle-closure glaucoma with no Humphrey visual field defects,ocular diseases and high or low blood pressure, diabetes etc, whose rectificative visual acuity ≥0.6; 32(32 eyes) patients with acute attack stage of primary acute angle-closure glaucoma ,examining after medicating and surgery 15 d, without using medicine IOP<21mmHg, rectificative visual acuity ≥0.4, without high or low blood pressure , diabetes and ocular diseases excluding glaucoma eyeground defects. Then examining at 1m(31 eyes) and 3m(28 eyes) after surgery, whose IOP keep normal. We still collect 15 normal subjects (30 eyes) , including 7 men and 8 women; age 41-67, median 53.27±8.83 years. whose rectificative visual acuity ≥0.6, with no ocular diseases and high or low blood pressure, diabetes etc. All collected patients and normal subjects underwent F.A.C.T(Functional Acuity Contrast Test) 1.5, 3.0, 6.0, 12.0 and 18.0(c/d) CS and color vision tests. Using SPSS11.0 did Statistic analysis.Results:The threshold of CS at 1.5 c/d, 3.0 c/d, 6.0 c/d, 12.0 c/d have no distinct difference in the preclinical stage patients .The threshold of CS at 18.0 c/d was increased in there preclinical stage patients (P<0.05). The preclinical stage patients with abnormal color vision was 20 eyes (about 57%); The normal subjects with abnormal color vision was 4 eyes (about 13%). The difference in two groups was significant. The threshold of CS at 1.5 c/d, 3.0 c/d, 6.0 c/d have no distinct difference in the patients after surgery 15d .The threshold of CS at 12.0 c/d and 18.0 c/d was increased in the patients after surgery 15d (P<0.01). The threshold of CS was higher in the patients after surgery 1m than in the patients after surgery 15d, but the difference was not significant(P>0.05); Comparing CS of the patients after surgery 15d, 1m and 3m ,the difference was not significant. The patients after surgery 15d with abnormal color vision was 23 eyes (about 72%); The patients after surgery 1m and 3m with abnormal color vision were the same. Discussion:Many studies show that before the visual field and optic disc defects the damage of optic nerve has already occurred. As clinical retina nerve fiber damnification occuring , there is about 50% nerve fiber damnification in tissue pathology examination. Visual field defects also always posterior to retina nerve damnification. For visual function damage of glaucoma was not restorable, the early diagnosis and therapy become more important. CS is a important measure for visual function. Commonly believed CS is one of the most sensitive measure for visual function. In clinic CS examination include three kinds: raster charts or raster cards. This technique is comparatively easy, but not exact and only can mensurate tiptop to 6c/d.
