Objective: Today date, it is not idea skin substitutes for treating large wounds by burns. Acellular Dermal Matrix (ADM) were used in the treatment of patients with full-thickness burns,which may be a better method.But many of questions about ADM have not been clarified,which make stop to use ADM.We explored the mechanisms and efficiency of ADM as dermal substitute in reconstruction of full-thickness skin defect by comparing two different ADMs.Methods: Meshed and granule allo-ADMs were transplanted on full-thickness skin defects on the lack of swines,covered by split auto-skin.In controlled group,only split auto-skin was grafted.The survive rates,contraction,histologic changes and revascularization procedure were recorded and analyzed.Results: All the grafts of the three groups survived.Compared with the controlled group, there were much less wound contract(P<0.01),better elasticity and less scar in the two experiment groups with meshed and granule ADMs.Conclusion: Meshed and granule allo-ADMs can be used as dermal substitutes in reconstruction of full thickness skin defect. Granule allo-ADM is easy for preparation and application, and may be a new means to reconstruct dermal defects in full skin loss.
