| Objective To determine the effectiveness and safety of hysteroscopic TVRC for the diagnosis and management of cervical lesion, and comparate pathology between TVRC and colposcopic multiple biopsies and cervical .pap smear. Methods Colposcopic dignosis and TVRC was performed in outpatients of 218 patients with cervical lesion, take on retrospectively analyzed focusing on the comparative study in pathology and .pap smear, by self-contral method. Results Among the 218patients, after TVRC pathologic report chronic cervicitis in 110 patients(50.46%); CIN I in 55patients (25.23%); CIN II in 31 patients (14.22%); CINIII 级 in 16 patients (7.34%);early stage cervical cancer in 1 patients (0.46%), 5 cases (2.29%) of resection marigin undertermined ( because of distroied by electrothermal in cell which influence pathologic diagnosis) . 167 cases were performed TVRC and colposcopic biopsies at same time, there was a correlation in pathology between them in 88cases (52.69%) .Of the 86 patients,there was correspondence between pap smear and colposcopic multiple biopsies in 29 cases(39.19%) .. During the mean follow-up time of 18 months, no cytology abnormality was detected.in 126 cases. Human papolloma virus-16,18(HPV-16, 18) was detected in 95 patients, infection in patients 2l cases (22.1%), they all became negative after TVRC.Theoverall cure rate in this study was 119 (94.44%) after first TVRC done, After TVRC simple hysterectomy was performed in 1 case (0.79%) because of cancer in situ , only 7 cases that re-TVRC were performed because of big erosion areas, then were cured. Conclusions The advantages to use the TVRC to management of the cervical lesion include its simpleness, safety,short operative time,less bleeding,and high cure rate. TVRC could also provide pathological sample for diagnosis.It had high clinic value for the treatment of cervical lesion. |
