Progressing cerebral infarction is a frequently type of cerebral ischemic diseases .It would easily develop into totally cerebral infarction with irreversible neurological dysfunction, if the treatment is not appropriate or in time, which will threaten the life and rehabilitation of the patient.The mechanism of the progressing cerebral infarction is mainly as follow:(1) the extension of the artery thrombosis in the primitive location causes new stenosis and occlusion of the previous cabined blood vessels, or block the branch circulation;(2) on the side of severe arteriosclerosis, with or without ulcer or /and stenosis , at the beginning the thrombosis could not cause any symptoms, but by the extension of the thrombosis and the narrowing of the blood vessels, the infarction area expanded and the symptoms aggravate; (3)the cerebral infarction area was not enlarged but symptoms were progressively worsen because of the cerebral edema;(4)accompanied by infection, electrolyte disturbance, acid-base imbalance deteriorates the symptoms. Of which the constantly extension of thrombosis is the mainly reason of the progressing cerebral infarction. And now, the most important problem is what are the dangerous factors that cause the thrombosis progress and neurological function deterioration, how to prevent the thrombosis from expanding.To discuss the mechanism of the progressing cerebral infarction and analysis the risk factors, explore the method of controlling this disease and improve the prognosis, this study made a systematic retrospective review of the 242 patients with infarction of the cerebral median artery, whose onset duration were between 6 hours to 1 week, from 2001 to 2004 at the department of neurology in China Japan Union Hospital of Jilin University, and studied the effects and safety of combining- treatment of Batroxobin and LWMH in progressing cerebral infarction. 242 cases with acute progressing cerebral infarction within 6hours to 1 week of onset were randomly divided into three groups : groupâ… (with combining-treatment);groupâ…¡(Batroxobin-treatment only);groupâ…¢(LWMH-treatment only).The clinical information of the progressing and non-progressing groups were also respectively collected. Compare all the progressing and non-progressing patient with blood sugar, blood fat and fibrinogen, and analysis of the risk factors of the progressing cerebral infarction; compare indexes of blood coagulation and fibrillolytic system examined before and after treatment of all the three groups. And according to the neurological dysfunction grade criteria of stroke drew by the 4th national cerebrovascular disease meeting, the patients were marked by the criteria before and after treatment and were carefully compared.Through the analysis, it was discovered that the average index of blood sugar, glycerol trimester, cholesterin and FIB of patient with the progressing cerebral infarction were apparently higher than the non-progressing patients, while the risk factors, as hypertension, hyperglycemia, repeat TIAspasm, fever, were also higher than the non-progressing patients, all these analysis outcomes of these risk factors have the statistical meanings. So when treating acute cerebral infarction, special attention should be paid to the patient with sole or multiple risk factors to avoid the occurrence of progressing cerebral infarction. At the meanwhile, the bad control of risk factors succeeded with bad treatment effect of the neurological function. So we should pay attention to control the risk factors while dealing with cerebral infarction.
